Funny that no one thought of a teleporter BLOCK, instead of an item. Oh well.
It would be crafted like this for 2 pads:

The pad can be placed on the floor. It has 3 inventory slots that determine it's frequency. When two teleporters have the same frequency, powering both with redstone causes blocks/players up to 2 blocks above the pads switch places once(blocks on pad one move to pad two, and vica versa.)
More than two pads with same frequency will rotate the blocks(pad1 goes to pad2, which goes to pad three, which goes back to one) clockwise relative to someone looking north.
Teleportation is often shut down because "it doesn't suit Minecraft". However, I think with Enderpearls, we may have found a way for it to make sense for Minecraft.

how about to stop the flaming it can only be used at a certain distance like 50 blocks because a nether portal can go up to 500 in 30 secs of walking to the pad can be used as a jump for mines and towers ect

It teleports ANYTHING 2 blocks above it, Entities AND blocks. This is a lot more useful for transport. Furthermore, it switches the two blocks above with the connected pad's blocks, so you have to worry about that.
Speaking of which, I think I know a better, more private and secure way of doing it. Why not have a pad have a GUI with a name slot and a map in it(What shows depends on what got explored on it) with other teleporter's locations and names on it? Yes, ones in the nether are shown. You link the teleporter you're accessing by clicking on any other pad on the map. You can only delete the new link from either pad. This means that a SMP game doesn't have the dangers of players accidentally or intentionally ruining other people's set ups by setting up a pad with the same "frequency".
