I have literally a dozen ideas for realms, but to keep this short I'll only post 2 of them here and discuss my takes on ideas already posted in other threads. I'll also break this thread up into 4 posts, with the realm being discussed in bold at the top to make sorting through this easier.
First off:
The Nether
On the nether, I'm going to make a short moddification of sabata2's suggestions, originally posted here.
When I first entered the nether, my first thought was "awesome!" Anyone else get this feeling?
Notch intended to make the Nether Minecraft's version of Hell; in fact, he originally referred to it as a "hell world" or "hell biome" (source). Yet the current Nether is simply a world of fire. To truly make it a "hell world," it must sap all hope from the player. A desolate and lifeless world of flames and destruction, any trip into it should be a suicide mission, even on peaceful.
Sabata2's vision is an ambitious project for modders, but the man-hours it would require would be prohibitive to implement into an unmodded version of the game. What simple things can Notch's crew do to update the Nether so that only the bravest players will dare enter?
Pillars of flame- eternally burning towerss of fire spouting from the walls and floor.
Fire jets- spontaniously appearing pillars of flame that last for a few seconds then subside.
Debris- perhaps a form of weather for the Nether, having blocks of burning rock fall from the ceiling and explode on impact.
Earthquakes- shake up the player and any non-floating mobs (but don't affect the environment).
New highly-aggressive fire-based mobs, such as flame/lava golems, red drakes (miniature, fire-breathing dragons), smoke elementals (ghosts made out of smoldering ash and smoke), etc.
This is only a short list of suggestions. There are hundreds of other things that could be added to the list. In fact, Notch could even work with modders to implement updates like sabata2's into the vanilla version of the game should he so desire.
Later-on, if he starts adding boss monsters to the game, he could even spawn a fire-god into the realm to make even the most courageous players shudder in fear.
There isn't much that could be done to make an earth element-based world that hasn't been done in the overworld already, however I do have some ideas.
A rugged, mountainous and plant-less world full of intricate, almost endless cave systems, and abundant ores (possibly even new ores not found anywhere else). Include the standard rock golems, tunneling mobs (molepeople, giant worms, etc.) and it could be an amusing place to explore.
Perhaps the addition of earthquakes and an increased instance of gravel (for cave-ins) could make this realm seem more realistic, but earth is always considered a slow, unchanging element. Such a world could get boring very quickly to a vast majority of players.
This is the one I've put the most thought into. The addition of a dark and ice-filled realm, resembling Niflheim from Norse mythology.
Lowered visibility, constant snowstorms, strong winds, lack of any solid ground (ice, water and snow blocks only) and aggressive frost-based creatures who's attacks can slow down the player's movement would make this an exciting (if dangerous) place to explore.
Adding some form of placeable ice block would give players a whole new method of creating masterpieces and the addition of a cold, blue fire as a method of light would only serve to increase the aesthetic value of the place.
I read the rules and did my best to avoid any rule-breaking, so I'm not sure what I did to warrant such hostility. If you can supply a link to explain what I did, I will work to rectify the situation and avoid it in the future.

Notch has already stated that he has ideas for a Skyrealm (source) and GreyAcumen made a very compelling case for his Aether-realm here.

Curse PremiumWhy does there have to be anything to gain? Most undertakings in minecraft are usually done just because the player can, are they not?
But I see your point. Maybe the addition of unique items such as flame-based weapons that can light the target on fire?
