The "Danger Detector" works like a compass or clock, that is its useful in its display.
The item looks like a remote with a square in the middle. The square changes from GREEN to RED as the player gets closer to lava, cactus, (upcoming) spike blocks, fire, monsters, etc.
It would server as an early warning system for players. It would only be able to detect one danger though. What danger is determined on crafting.
Recipe could be something like
Where the lava bucket can be replaced with cactus, spike block, gunpowder (for creapers), feather (for zombie), flint&steel (for fire), bone (for skeletons), string (for spiders).
The detector would be an indicator when whatever was used to make it is within 10 blocks of the player.
IF the range is much larger than 10 blocks, it would be going off perpetually, making it virtually useless. If it detects less than 10 blocks, it's going to be useless because you should be able to see and/or hear danger in that range without assistance.
If you really want to detect creepers behind you, put some dirt down before you start mining, you'll hear their footsteps.
Inventing an item that is only really useful for making creepers less dangerous seems like a poor idea.
A cactus detector seems patently silly, and a spike or magma detector would make it really easy to circumvent even the most convoluted traps.

I Can't Sleep on The Ground You Know...
It would be nice to know that i have a creeper approaching from behind. And 10 was just me picking a number out of the air.
