So here's a new idea I came up with: Hardcore mode. A thought of this because a lot of people (including me) think that Hard mode is too "easy". Selecting it would be just like any other difficulty setting (going to options and changing), so it would be completely optional.
All damage would be like the current hard mode, so I guess it would sort of be like a variant. There would obviously be some key differences though. One would be a hunger bar. Now I know that there's gonna be a hunger bar in update 1.8, but I don't expect it to be "too much" as most of the stuff in the game currently make the character like some sort of super soldier or something. Well anyways, this hunger bar would depleat faster/slower depending on what activities you do during the day. So if you like, mined all day, you hunger bar would depleat faster than if you just spent the morning picking flowers. See where I'm going?
Another difference would be that food would no longer heal you. I mean if you were almost torn apart by a zombie, have half a dozen arrow wounds, several giant spider bites, and you two front legs broken from a creeper blast, just stuffing some cake in your face isn't gonna cut it. Now I know that minecraft isn't supposed to be realistic, but c'mon, this is "hardcore" mode. So yea, while food will be nice to your hunger bar, it won't do anything to your fatal wounds. Well actually I was thinking that maybe golden apples would be the only food to heal both health and hunger as they are pretty rare, but I'm not sure. Anyways, I havn't really thought of a way how the player would heal their health, so maybe you guys can help? One idea I had would be a type of medicine made from flowers or something, but I'm not too sure.
Along with hunger, there would also be a thirst bar. All you have to do is go up to a water source block and right click or something (maybe jump into it?). The thirst bar would go down slightly faster than the hunger bar, but you could easily refill it via a water bucket.
An idea I was sort of playing around with was high/low altitude breathing. You know those breathing bubbles that appear if you go under water? Well, if you go too high up on a mountain or something, those same bubbles would appear. They would of course, deplete much slower (maybe one heart every 1 or 2 minutes). Just taking a quick breather lower down would completely refill the bubbles, but I was thinking of a sort of helmet item like an oxygen tank? The same thing would happen if you go too deep underground, but it would be harder because your underground, and you know...

Not a bad idea. Golden Apples should definitely heal you (fully). I like the high alt thing but it could cause issues with people who build really tall structures or sky-scrapers.
One good idea I have:
You couldnt just change the gamemode to peaceful if you get scared of death. When you create a new world, there would be "Hardcore" thingy, so you wouldnt be able to change the difficulty in-game.
Also, there wouldnt be respawning. Your world would be gone.
Ive seen clips from Indev, but I wasnt playing Minecraft back then.
What it also needs is more food. Like berries and other **** from forests.
E: What I meant with the "Hardcore thingy", is that if you check that, you cant change difficulty in the world you just created. In other worlds you still could do that, and respawning would be in the original difficulties.
Or maybe there could be like a super difficult achievement where you have to find diamonds in a world without turning Hardcore mode off once?
Perhaps this Hardcore difficulty should be an option in making maps. Also, Hardcore maps should give players rewards for stepping into danger, such as guaranteed dungeons on mountain summits and deep water.
