If you step on them, they bite you, and deal about 2 1/2 hearts of damage, and then, after about 15 secs, you take another heart. Later, another. And another. And another. The only way to cure yourself would be to eat food, which doesn't heal you but only cures your poison.
The only way this could become a threat, though, is to make them hard to see.
They would hide in tall grass without moving, and look like they're made out of green-tinted glass. They would die in one hit, though, and drop poison sacs and occasionally (25% chance) 1-2 gecko eggs.
Poison sacs could be used to poison your weapons, making your weapons much more deadly, especially in SMP.
Eggs would be like chicken eggs in that when you throw them, they have a chance of spawning a baby gecko that you can pick up and put in your inventory, and if you held them and right-clicked, they would follow you around and help you fight. They would only do 1 heart of damage, though, and their poison wouldn't be as strong (1/2 heart per 20 secs)
They would also be bright orange.

Except you probably wouldn't be able to have pet urchins, so maybe they just drop poison sacs.
