Look:
Like a bullfrog stand on it's hind legs, wearing tan linen pants with a brown rope strap around the top. They wield Platinum swords and have a fly on there back. About 1/4 block in height, length, and width. 1 block long when counting the sword.
Sounds:
First Phase: Makes soft ribbit sounds or buzz sounds when the player is unnoticed. Soundless when played is noticed but still not attacked. Louder ribbit sound when player is being attacked.
Second Phase: Loud buzzing noises.

Any place that is with 5 blocks of water and is as dark as when it rains during the day time. Has a 20% chance of spawning in these areas. Usually found near lakes or oceans during night, or during or after a thunderstorm or rain. Also usually found near small underground pools.

1st Phase: Same speed as player
2nd Phase: 150% faster than player

1st Phase:
When Not Provoked: Stays near water usually not venturing towards hazards or light. When in light will search for dark immediately. When not within 10 blocks of water from 30 seconds to 2 minutes will go to nearest. Will sometimes purposely go into water when have not been in a while. When a player or mob is heard within a 75 block radius will get out of water and wait from 5 to 7 1/2 minutes.
When Provoked: Will try to find a way behind the player in the darkness to try and ambush player. If the player is within 5 blocks will jump towards and try to get to the back while attacking. When on higher land and within 3 blocks will jump near back of player and strike. *The frog will disappear when killed and the fly will have the sword under it.
2nd Phase:
When Not Provoked: Will try desperately to get to dark. Once in dark will stay there until Provoked.
When Provoked: Will try to fly away from player desperately. Will sometimes buzz giving all mobs with a 20 block radius a 25% of walking in the direction of the player.
