I manage my own server and from what I have seen is ~80% of all maps are of the same area (around spawn) whenever I do not spawn maps (all the same damage value) for the clients. So I propose to have sector-ized maps. For instance, when the world is generated, it overlays a grid with each square of the grid being the size of a map. Each grid square would only have one map tied to it. Whenever someone makes a map, it will clone the map that corresponds with the grid square the map is being crafted in or create a new map for that grid square if there is no corresponding map for that grid square. It would remove a lot of complexities with creating a map. There would not be any more shift-clicking to duplicate map_0 or inventory editing for other maps, and servers will not have hundreds of map files of the same area.
To be honest, I think the whole problem could be solved by using right click to set the center of the map. Of course it would prompt you with a yes or no to prevent doing it on accident. What could also work is using a certain arrangement of blocks (like Obsidian surrounded by grass with a cake on top in the center), as a corner. Place four of these, right click with the map anywhere within the boundaries of this square, and you have a map of the area within the square, zoomed to your liking.

I have to speculate though the reason notch and jeb_ did not put any dialog boxes or menu systems into the game upon creation of the map is because it is overly complex and unintuitive. Also, the idea was geared more towards cooperative play where people would not have a reason to hide. Anyone could get a sector map simply by making the map in the appropriate sector thus allowing all players in that sector to be visible. This idea would not be applicable for PvP servers without a team system. On PvP servers with team systems, each team would get its own set of sector maps (but that is completely separate idea).
How about every map made from the same crafting box will be duplicates of each other. If you don't want to be seen on the map you can just move one block away and make it.
