I thought of most of these ideas in the shower... I need to shower more often.
This is a list of many ideas I have that all connect to the realistic mode that may come sometime.
I: Meters
1: Temperature Meter.
Knowing how cold or warm you little guy is would be a must for realism. A desert could make your guy warmer, lava could make him too hot, snowy lands could make him cold. There are items that will control heat levels and I will talk about those in the next section. Being too hot or cold will make him die of hyperthermia (heat) or hypothermia (cold). The bar is 40 thin rectangles (blocks), with 20 on either end.
2: Hunger Meter: Eating is a necessity in the world. If you go too long without eating, maybe 7 Minecraft days, you will starve to death. Possibly each day you go without eating you get slower, lose energy (will talk about later), and lose a heart due to exhaustion. Eating extra food that replenished more hearts than needed goes to your hunger meter to keep your bar lasting longer. The hunger bar is 10 blocks long.
3: Thirst Meter: Drinking is also a necessity. You would need to drink at least every 3 Minecraft days or you will dehydrate. Quenching thirst would be done with new items, I have many items coming along too. Heat, like in the desert, would deplete your water more quickly than usual. The thirst bar is 10 blocks long
4: Energy Meter: Sleeping is the 3rd of 4 necessities I will discuss here. Not sleeping can cause you to die, it has been proven. You might need to sleep at least once every 5 days, and this would allow you to sleep whenever if you have low energy levels. Heat can exhaust you slightly faster. The energy bar is 10 blocks long.
5: Air Meter. Breathing is the last necessity I know of. Down in deep caves, ammonia levels can be quite high, which can disrupt normal breathing. Being up very high where oxygen is thinner can also lower breathing ability. If your level is too low, you die. There are items to fix this too. The air bar is 10 blocks long.
II: Crops
1: Cocoa Trees: these are short trees that have normal log (not birch or pine), and brown spotted leaves. Clearing leaves would have a chance of giving a sappy, a cocoa bean, or both.
2: Apple Tree: In singleplayer, it is impossible to get a red apple, since only Notch drops them, and gold apples are extremely rare in chests. Apple trees would have normal wood, and the leaves have large red spots. Golden apple trees can only be made by surrounding an apple tree sapling with gold blocks, and they don't drop any more golden apple sappies. Only golden apples. Red apple trees have a change of dropping a sappy, and apple or both, and golden apple trees only drop golden apples.
III: Drinks (no new food)
1: Chocolate Milk: This is a canteen (get to that later) of milk put into the crafting menu with cocoa beans.
2: Hot Cocoa: This is considered the ultimate drink for cold climates, it is made by putting chocolate milk in a furnace.
3: Slushy: Taking a canteen of water and putting a snowball on the crafting board with it will give you a slushy. Ultimate drink for hot places.
IV: Tools
1: Canteen: This is pretty much a water or milk holding bucket, but you can drink it. Instead of placing liquid, you drink it.
List of liquid effects:
~ Water: Gives 5 blocks into your thirst meter.
~ Milk: Gives 3 blocks into your thirst meter and heals 1 heart.
~ Chocolate Milk: Gives 3 blocks into your thirst meter and heals 2 hearts.
~ Hot cocoa: Gives 2 blocks into your thirst meter, heals 3 hearts, and pushes the heat meter 5 blocks to the warm end.
~ Slushy: Gives 4 blocks into your thirst meter, heals 1 heart, and pushes the heat meter 5 blocks to the cold end.
2: Oxygen tank: Allows for breathing in high elevations and deep caves.
3: Hot Cocoa: + (leather helm = chocolate milk canteen)
4: Slushy: (diamond is a snowball, helment is a water canteen)

Leaves will make oxygen.
7: Wool armor: same as current armor, but with wool.
8: Cold wool armor: Putting a snowball and any piece of wool armor into the crafting table.
I'm pretty sure you can think of some more, because I really have to go now, and I'm really serious, so tell me what you think of everything and the general "realistic mega update" as a whole.
Like any other difficulty, realistic would be an option.

Very rare though, and dungeons are hard to find, to stack on top of that. Apple farming would be nice. Cocoa farms would be a joy.
