If there is no torches in a deep cave you would get cold.
If there is no torches outside and it is snowing your obviously cold.
There should be like an set of armor that is like a wool coat, boots, pants, and hat that makes you invincible to the cold.
Could be crafted with any color wool. The wool armor would have the durability of leather or a little less. And the piece of armor would be the same color as the wool made to make it. (Black would look sweet!)

I kinda like the idea of having to deal with environmental issues, such as freezing, when cave delving. And an actual fireplace for in-game would be sweet....but you could have the same effect with a furnace, right?
I mean, what you outlined out was basically a furnace with a flint&steel...why? A few thoughts (yeah, I may be a jerk, sorry). Why not make a 'fireplace' out of either stone or brick, increase the timer for burning items such as coal, wood, charcoal, etc. based upon the materials.
Example: in a furnace (or smelter) a lump of coal lasts about 40 seconds (okay, okay...I suck on the details), however when the same out line is used with stone or brick....

Here's the big catch....these fireplaces, stove or Iron Furnace cannot be used for smelting (stove can be used to cook...not sure how that would work out coding wise), all they provide is heat (and light in the case of fireplaces).

The idea of freezing, though already suggested, is a cool one. I think that needing specific armor to keep warm would be annoying, as you would need to switch to real armor before combat. Perhaps if any type of armor kept you from freezing. And this idea would fit much better in realism mode.
Wouldn't you get warmer the farther down you go in a cave as you get closer to the earths core? I mean if you get down to lava pits, you would not be cold at all. Also, many cultures use underground houses as they are easier to keep warm. Oh and the fact that going down into a cave in Minecraft without torches would be a very stupid thing to do already. I would like you to have to have a heat source in snowy areas though (realism mode only)
