The item would look like gunpowder or clay, be lime green, and have a radioactive green tint to it.
Uses:
Anything poisoned would cause 1/2 heart / second after other damage is dealt.
Indication of poisonous weapon would be: (all in the item box)
Idea 1: A bar above the weapon for 2 uses, would be purple.
Idea 2: Have a skull and crossbones behind it, after 1 use, half the S&C would go away, then after the second, the other half.
Idea 3: Have 2 separate boxes in the top of the box above the weapon, (purple), have them individually go away after each use.
Indication of being poisoned:
Idea 1: Have a skull and crossbones above your hearts.
Idea 2: Have the screen shake as the poison hits every second.
-Combined with arrows to make poisonous arrows. (1 arrow + 1 poison = 1 poisonous arrow)
-Combine with a sword/other tool to a two use poisonous hit. (1 sword + 1 poison = 1 two-use poisonous sword)
p.s. sword/other tool would go back to regular after poison has been used.
-Combine poison and bucket to make a full block of it used for poison traps. (Have each block of poison spread 2 blocks in each direction) (2 poison + 1 bucket = 1 Bucket of poison)
And of course if there's poison, there's an antidote. It could be either simple or complex:
-Complex: Have a concoction of a bowl, mushroom ( OR ), and flower ( OR )
-Simple: Just eat a mushroom ( OR ).
Additional notes:
If there were to be poison, I would think that Skeletons should be able to shoot poisonous arrows: All the time in Hard, 50 poison/50 regular in normal, none in easy.
Or spiders might have a 25% chance to poison you.
Snakes would have a 50% chance to poison.
Same as snakes for Scorpions.
One thing left to figure out, is after the arrow is shot and missed the target, should it revert back to a normal arrow or remain poisoned?

This is one of those ideas that has been suggested many times over. It's not a bad idea, if poison is your thing, but I would suggest searching so that support can be gathered in fewer threads.

The Poison would be on each arrow, hence 1 arrow and 1 poison make 1 poisonous arrow. Every arrow would need to be dipped into one poison, making one poisonous arrow. This wouldn't apply to the bow although it would be able to shoot the poisonous arrow. The reason why it wouldn't be a bow is because the poison would be a renewable material found in nature. I get what you're saying though, about the balance. The only thing that's left to figure out is after the arrow is shot and missed, should it revert back to a normal arrow or stay the same.
