first off most of the threads i looked at were about making hostile mobs harder and tougher.
this doesn't really add to diffuculty all that much. you just hide and mine and repeat.
set difficulty can still be changed to easy, normal, etc the map design just changed
this thread is about making actual resource obtaining harder through just changing the world design without needing to make harder mobs.
when in the seed generator for a world you could type in something like tricky or difficult. this would spawn you in a world completely made of floating islands. everything will stay the same though, skybox might have to be changed tho.
the islands would be at max 25 blocks deep and would contain all the resources if your lucky. this limits mining space causing for more risky attempts. also this means you can mine your spawning island out of existance. by scattering vital resources far and wide your first night can get a lot tougher with only 4 trees and a island going only 8 blocks deep. this causes for more wise resource making decisions regarding tools and such. furthermore this makes already hard to get things such as lapis and diamond even harder to get since island space is limited.
falling to you death: this needs to be cleared up. there is a absolute zero which the bottom (158 from the top of the map) of islands only reach 30 blocks above it. once you hit it your dead losing all you stuff like a void, except you respawn. so if you fall off an island you can die from falling damage on another island or fall into the dead zone.
