As the title implies, most people who complain about wanting their name changed do so because they simply didn't know that account name = ingame name. This is unlike most games today, where account names in WoW, Steam, Ultima Online, etc do not translate directly as your ingame name.
A simple "This will also become your ingame name, and cannot be changed" somewhere on the registration would fix most of the issue.
...sounds more like a problem with people not reading the warning when you apply for membership.

Just logged into a non-registered account to attempt to buy membership. Not seeing any warning.
