The Nether is too easy. For me, it's like taking a trip to Monaco. I think it needs more difficulty. Zombie pigmen are friendly unless attacked and ghast fireballs are easy to avoid. The only real threat I find is falling into lava and losing my items.
... Which is why I suggest demons. Demons are hostile mobs that attack you relentlessly. They roam the Nether in packs of two to four. They will come down hard upon you when they see you and they will attack you even if you haven't attacked them. They're slightly rare, but still a common sight in the Nether. They use their hands to attack you, like zombies.
Stats (on normal difficulty)
Attack: 3 damage
Health: 10 hearts
Speed: Normal
Special abilities: Leap attacks
That's right. Demons only have average speed, and they overcome this with leap attacks. Using their wings, they leap into the air to attack you, similar to spiders. They take no fall damage so they can even leap from high places to get to you. They don't fly, though, so they can't reach you if you're higher than them. They don't spawn in a light area of 10 or higher, so you'll have to place torches in your Nether home. This does not apply to fire, lava, or glowstone so you'll have to specifically use torches in your home to keep it demon free.
Demons have one good thing about them: They drop golden apples. Killing a demon will make it drop 0 to 2 golden apples which will heal you completely. Demons also drop 0 to 2 obsidian, diamonds or glowstone on rare occasions.
There is one other type of demon, the archer demon. This demon has his normal attack replaced by a bow and unlimited arrows. The arrows are lit and they do 3 damage. The demon archer has the same accuracy as the skeleton, and for every demon spawned, there is a 10% chance that it will spawn as an archer demon.

I like the idea of adding some new features/mobs to the Nether to make it a little more interesting, but I'm not sure they should drop so many golden apples. That seems a little...much. Maybe if there was a chance of them dropping 1 golden apple then okay, but not if they dropped 1-2 every time. It could be possible for them to drop something new as well, if there's anything Notch has been wanting to release, but hasn't been sure what to make drop it. Maybe something you could take to temporarily increase the damage your weapon does by a certain percentage?
Also, it could be neat if they could "enrage" all the mobs within a certain radius of themselves, causing any nearby Pigmen to become aggressive and come to attack you as well? I think it would make sense if a demon was able to turn a normally neutral mob "Evil." But that might make them a little too difficult to deal with...

That's a good idea. I'm not quite sure what item could act as a replacement for golden apples. Golden apples work, but they seem more like an overworld thing. Maybe if demons dropped obsidian, instead.

I see what you mean. Ghasts are possible to avoid even when 10 are shooting at you constantly, and only if you want cooked pork without the cooking you get on the wrong side of the only other mob there. I support having more mobs there, but maybe some other mob? Demons are unoriginal.
