I was thinking about an extremely hard level of difficulty. Couldn't really name it "hardcore" because "hard" is already there.
In extreme, you must eat every minecraft week, or you will slowly feel weak by walking slower and (very slowly) losing health. You also need to drink every 3 minecraft days (this won't kill you as quickly, you'd lose health every day, letting players get some iron to make a bucket and drink milk.)
Skeleton jockeys have a higher rate of spawning (5%) and skeletons themselves have a higher accuracy level.
Zombies inflict hard-level damage but if you're wearing leather or no armor, you have a chance of being poisoned. With leather armor you need to have at least 25% of the armor's durabillity down before having a chance of being poisoned. When unprotected you have an 80% chance of being poisoned by a zombie.
If you want this to be added, you need to make an item...
The vial is simple. Put glass blocks in a bucket arrangement.
Zombie poison does several things. It drains your health (for 100 seconds), slows your speed (2 minutes) and disorts your view (Time ranges from 3 to 5 minutes). The antidotes? = Vial. and are all in buckets. for damage antidote (Sprite appears a purple-ish liquid quite similar to obsidian. Cures damage of course. = Bucket of milk. creates the disortion antidote which, of course, removes the annoying disortion effect.
And finally, the speed antidote.
ALL recepies can be made backwards.
Spiders have the habit of climbing walls to both avoid your attacks and push off to attack you.
Creepers that see you will make a rough estimation to your path and hide nearby. They become smarter and learn where you normally go.
Slimes have become smarter as well and tend to stay still to ambush you from a corner.
And finally...
HE (you should know who I mean by that) will appear now and then taunting you to make an attack. Attacking him pushes him back, and his eyes glow red for an instant. Then he moves with blinding speed, ramming you as an attack. If he happens to ram you, you'll be stuck in a sort of "cutscene" where he holds your neck high in the air with eyes still red. The screen will inform you to click rapidly to break out of his grasp, losing health every 3 seconds. The screen slowly turns red and disorts your view like the zombie's affect of poison, though this one cannot be cured with the antidote. When the player has clicked enough times, the player kicks him and then the regular game goes on.
Other times, he'll appear and make something similar, taunting and glowing red eyes. But he uses another form. Strength. He's the size of The Hulk! He'll smash any obstacle in his path at his attempt to grab you. When he grabs you, he puts you in the air for an instant, 50% chance throwing you or the other 50% acting L4D-tank-style on you, but instead of smashing, pins you to the ground. Like the choking scene, but requires more clicks. After breaking out of the grasp, the fight continues.
If you happen to beat both forms, he tries to keep himself hidden, but might still appear.
Before saying stuff like "Dude this idea is shitty I can't believe u thought of this" or some immature trolling like that, keep in mind that you HAVE THE OPTION to turn it off or play regularly.
I also treat people simply saying "TL;DR" as one-word spammers.

Basically, the idea is; No. Has been suggested several hundreds of times before, and I'm not exagerating at all. Just use Google Search to find "Extreme" "Harcore" or "Insane" Difficulty, and you'll see. Or at least you should.
Also, Hunger will eventually be added. Thirst too, perhaps. But in a "Realistic Mode" only, not in the main game. Until then, just wait.
I quite like the idea. Adding basically a realism difficulty to the game.
Maybe not a new difficulty level as such. Just another box which you can select it on or off.
So, Peaceful + Extreme = Need food every 14 minecraft days + Water every week.
Then gradually get harder.
So, Hard + Extreme = Need food every 7 days + water every 3 days.

Yes I know Notch is going to add "Realistic Mode" but please note I said you have the option to turn it on or off. And in realistic mode, it just adds hunger and thirst, there's plenty more in here.
I'm not saying this is better, because that gave me the idea of this.

I relly like this idea, but starving/dehydration should only just take away health. I would say lose one bar each day, and one bar each night. That is all... Oh, and enemies do realistic damage, where, if a zombie bit a chunk out of you, it's going to hurt. If a skeleton shoots you twice, you're dead. If a spider jockey is after you, you're screwed.

Hmm... Maybe, but if you're lacking what you need to survive, you'll feel weak. It doesn't slow you down as much as zombie poison. If the Regular walking speed were 10, it would be around 8.5, where zombie poison takes you to 5.
