Right now, eating is simply a way to restore your health if you get hurt. I'd love to see it become necessary in survival mode to eat to survive. After all you have to do it in the real world to survive. I think it would work out well if once every "Minecraft day", you would lose "1" heart if you didn't consume some form of food. This would make it a part of survival to hunt/harvest food to remain healthy. It would also add to the "survival element" if eating uncooked pork or fish would entail a "1 in 10" chance that you could contract a debilitating sickness that would need to be treated by medication (a new crafted item?) that would eat away at your health at a faster rate.
Notch is implementing a realism mode, where starvation will play a part.
What about 'needs' such as *ahem* bathrooms *ahem*

..Then the people asking for hunger and illnesses will get exactly what they wanted. If we must make Minecraft realistic, then by God, let's make it realistic.
