1.Crafting
Crafting these would be pretty simple. Put 1 Yellow Glass Block on each side, and 1 on the bottom. Then you would put a nether star in the middle. Then, BOOM! You have your self a Forever Bottle.
2.Design
An empty bottle would look like a Bottle O’ Enchanting, but it has a enchanted effect, like the effect that would be on enchanted swords. It would also be more bluish then greenish in the middle.
3.Mechanics
It is as simple as any potion inside it would last forever, or infinitely.

They might be expensive, but they would break the potion economy.
I mean, they would provide a constant boost to a certain stat. Wherever you go. That's overpowered.
Unlike Beacons, which are just as expensive themselves but also require a ton of other kind of resources as base which makes them very hard to relocate and are very limited in their area-of-effect.
