To me food is semi-useless. If I am low on health and not exploring a long away from home, I just store my stuff in a chest and suicide. I think this is waaaay to easy, so I sat down and thought about how this could be fixed.
after a few bad ideas I come up with this that might just work:
when you die you spawn with 1-9 health (say 8 for easy, 5 for normal and 3 for hard).
also this would make it more realistic:
how it is now:
A skeleton comes by and shoot you. You die and magicaly wake up with full health at spawn.
how it should be:
A skeleton comes by and shoot you. you faint and wake up very weak at spawn.
Tell me what you think! =)
and if you are to lazy to write, atlast use the poll above.
Not too sure about this idea. I can understand what you're saying with the whole "If you get shot and die, you come back with full health" is weird, but fainting, moving location whilst out cold, then waking up with a bit less health sounds a bit weird too xD
Also, what if you fall into Lava or drown in water?
On the plus note, I agree that there should be some sort of penalty for dying or a mode where the penalties are high. I think there was a rumor about Mojang adding a 'hardcore' mode (you die, your world gets deleted) style thing, which I think would be best for this idea, as if leaves those who like the current re-spawn system to what they like and offers the players who want a lil more realism in the game to try and actually survive, literally, as long as they can.
On the note of food:
I find it useful because I reaaaally don't like traveling alllll the way back to where I committed suicide to collect the box of (most likely) useless junk when I could have just easily walked back the way I came, scoffing pork chops to get past falls/creepers etc. Suicide is most likely a personal preference and I doubt everyone does it that way.
But yeah.

you barely survive but manage to get yourself to the spawn.
and hardcore could work in singleplayer but not multiplayer.
there is also a difference between deleting the world and spawn with 1-9 health =P
this is an example of a normal day in multi for me:
hmm got a chest full of bread, 3 health and want to go on an adventure. should I eat the food I used 5-10 mins to get or just suicide and go back with full health in 30 sec?
I got my base near spawn like many do and the answer is always the same: suicide and spare my hardly earned food for my adventure.
