So like I would be wandering and find a small area a few chunk wide of nether materials where pigmen are wandering and glowstone sticks out of the ground? Sounds cool.
How's this then; pre-existing portals to the nether, deep underground, on the bedrock level. They would be like dungeons, at bedrock level, surrounded in bedrock rather than mossy cobblestone (or entirely made of obsidian).
Portal would constantly be open, and you'd have pigmen spawn out of the portal like a monster spawner.
I could see when the world generates a nether portal that it also generates a small cave of netherack around it. Maybe some glowstone and lava too. These of course would be just as rare as random portals are now. So basically instead of there just being a random portal spawned by the world there would be "corruption" as some people have put it.

So I suggest adding small pockets of nether in the normal world, to be found like the dungeons. but is very rare.
they may contain pigmen, but not likely. dunno about Ghasts.

you know you play too much Minecraft when you know that you removed a total of 7659 block's in your last play -session
It removes the only reason to go to the nether if you know how to make a good minecart track.
if your friend is building with nether mats in multiplayer, he is an op.
he can spawn all the stuff he wants.
the values are:
87: netherrack
88: soulsand
89: glowstone
Every step in the nether is as 8 in the overworld.

9x9 and 5 blocks high? Made out of what? With what lighting? Lava layers? How would the nether mobs spawn?
