Now, I apologize if this idea is too stupid, but anyways, moving on.
Right now, when mining diamond ore, you inexplicably get an entire, already-cut diamond gem. I'm fine with this, however. And since Notch is going to add Realistic Mode at some point in the future, I decided to post this suggestion.
If you have realistic mode on, mining diamond ore will get you a chunk of Rough Diamond, which is darker, grayer and looks more like coal. These uncut diamond gems are unsuited for crafting with, except for one little recipe.
Another new item related to this suggestion is the Gem Cutter, a tool crafted like this.

This tool doesn't work on any blocks. Instead, you craft it with a rough diamond, which gets you a diamond gem like in the current game, gets rid of the rough diamond and leaves the cutter in the crafting grid, but with some durability used up.
So how is it? Do any details need changing?

Just remember that none of this would be part of normal Minecraft. You'd have to be playing on Realism Mode/have the realism slider up to maximum for diamond ore to drop rough diamonds.

OK, one problem solved, but if this "realism" would that mean there would be no zombies, skeletons, spiders, or creepers, since the are really not associated with the word real, or realism?

Notch has hinted to adding a "realistic" difficulty mode, realistic as in the "you need to eat food, drink water to live" kind of realistic, not the "GUNS BLOOD CARS WAR SKYSCRAPERS AND HEAVY MOTION BLUR AND BLOOM EFFECTS" one you're thinking of.
