I was searching for this idea in the forums, and i was unable to find it, so here it is:
my simple suggestion is that Notch should add a hunger,water, and sleep meter. This would give the players a more survival feel, like they have to go out and find food or fresh water. Yes, fresh water, that means the player must find water that is inland (not the ocean) and drink that.
You can go-
5 days without food
3 days without water
and 7 days without sleep
If any of the meters fall below 0, you die, simple as that.
There could also be some more items such as:
a cup (to drink water)
a refrigerator ( I really don't know, im just brainstorming.)
and maybe a ability to sleep on the ground, but with 3 hearts lost, and the 3 meters to fall down more.
Because when I think of a game with cubic imagery, floating land, skeleton archers, zombies, and angry-exploding green penises; I am always reminded of its startling realism.
Also, people eat food too often for hunger to be a problem, infinite pools of water can be made easily and abundantly, and you can survive for longer than a week without sleep. You just grow kind of -STOP TOUCHING ME- batshit insane after the first 5 or so days.

Maybe too much water/food can hurt you
and that is were bandages come in, they don't recover hunger they HoT (Heal Over Time) your hearts like 0.5 hearts every 2 seconds for 10 seconds
and with Randalpiks mug suggestion, it could be sorta like the cups and milk and everything higher would decrease hunger a little, and decrease thirst more (you'll get it later)
Anyways I've been giving a lot of though into this recently, might post my ideas here
