The way I'm understanding...
It would look like a bit of an ogre or a caveman, hunched in appearance with big teeth and not much clothing and what is clothed is heavily torn.
It would do a lot of damage with his club... About 3 hearts... He would be really rare.
What are economics?

There certainly needs to be a large brutish humanoid in the game that is very tough to fight. I've been lobbying Mojang for something scary to run from for a while. :]
Trolls should be big-nosed, pointy-eared, and greenish grey. They are generally hunched over, but 20-50% larger than a player. They should spawn only underground, but not be harmed by the sunlight. They should be rare. There should be a chance of them having a sword and/or any type of armor (for using and as a drop). Most importantly, they should do x1.5 - x2 the damage of a skeleton and have x2-x3 health of a skeleton. Regeneration would be cool, but if that's the case, them being on fire should stop it. The drops should be the gear it's using (if any) and troll dung. That stuff can do whatever but I suggest it can be used to ward off creepers for an hour or so.
Also, it would be cool if they had bridges and collected tolls somehow, but that seems like a tough thing to code.
"They have a cave troll" came to mind.
I think it's an interesting ideo though.
Questions to consider: How big would they be? How many hearts would they have?
How would they behave? would they be neutral,agressive, or peaceful? Would they be able to interact with blocks?
What would it's animations for attacking/walking/whatever look like?

But this also means some of the random buildings generated should be bridges over rivers and chasms. With trolls underneath some of them.
