One(ish) suggestion: It's a little overpowered if it never lets go, period, so you absolutely HAVE TO engage. Perhaps if it olds for say 4-5 seconds then takes a short break and/or the arm part could be attackible, stunning it for 1-2 seconds after around 5-8 damage, letting you run in for the kill or run the Nether away from the thing. Maybe offset with a little more health.
It's just that if I'm fighting off a horde of ZPMs, getting grabbed by one of these seems like a "NOOOOAAAHHHhhhh $%%$&%%*#." kind of Natural 1 situation.
Not a fan of yet another player-movement-controlling mob, but this one is loads more sensible. Everything's well thought out and balanced, so I can fully support. This is just a small thing, but I'd recommend this thing dropping a finger rather than a whole hand; it's easier to brew just one finger in a potion. That's all subjective-me though. Does thing make any sounds?
You get an upvote just for the fact that you thought out the melee part, as this potion would have butchered PvP and SSP balance if this potion allowed extra melee reach.

Yup, if you bring a stick to a Reaper fight, you're gonna have a bad time, but this is the Nether we're talking about here; it's suppose to be scary. Just think of Soul Sand as a no go area, if you want to play it safe, because like Creepers, if you ain't prepared, you dead.

Mind you, these things will grab any living entity, which included Zombie Pigmen, too, and you should never anger a hoard of Zombie Pigmen, especially if you're gonna traverse Soul Sand. If that ever occurs, I recommend pillaring up with blocks instead.

I think Reaper is good because it accurately represents what the mob does, reaps souls. I think "Reaper" is a fine name, and if that is too much of a ghoulish, generic name, I'd maybe call it a "Soul Reaper" instead of just a reaper, however redundant.
I think this adds a lot to an empty Nether that we have. Although Mojang and others have been slowly working on the Nether, such as in 1.10, I think that it needs either a revamp or a complete overhaul. I think this adds to the atmosphere and purpose of the Nether, to be the last resting place of mortals.
