Well, I thought that, to add to the survival, is sickness. Now, this would be only on Normal and Hard. Normal it would be semi-rare, but fairly common on hard. Sickness could be spread many ways.
One, from animals. On both difficulties it would be the least common way. Killing animals would have a chance to get sick. You would know if you got sick (this applies to all sicknesses) the edges of your screen would be slightly green, and get darker as you got sicker.
Two, hostile mobs. This would be the most common way on Normal, second most on Hard. Hostile mobs (especially zombies) could make you sick just being near them. Killing them would spike the risk for about 1/2 a second.
Third would only be on Hard, and would be very common (still not too common). By staying in water too long would make you likely to become sick. Riding in boats would not have the risk of getting sick.
Of course, when you get sick, you need a way to cure yourself, right? Well, my idea makes soups useful as a food, as giving them curing properties. The three different sicknesses would require different soups.

if you are on hard.. after those you..
4. cough up orgins
5. head blows up because of super x-treme puking

what about instead you just become slower and your mining/attacking becomes much slower, and if in five days you haven't cured yourself... you explode. or die.
I was thinking that getting sick would first make you slower, then have your view as if you're sneaking, then tools become too heavy (in order: diamond gold, iron, rock, wood can always be held) to use.

So, erm.. something that punishes people for attacking animals/mobs? So if you dare to farm or go out at night, you better of found some mushrooms/ingredients? I'm not sure bout' this one.

Pretty much that seems to be the whole idea from what I can see.
Congratulations on getting your diamond sword, your reward is being able to kill five more cows than with an iron sword before you die from the disease you got killing the first one. That is if you do not die from the several hundred diseases you caught from trying to sneak up on a zombie, or from the cold virus you picked up swimming in an oasis in the middle of the desert.
But seriously, while I think it would be alright for certain types of mobs, say poisonous spiders or skeletons that shoot poison-tipped arrows to be able to poison the player, which in essence may have similar effects to being diseased, you should not be punished for attempting to fight animals and monsters or even go swimming for that matter.
well I disagree in general with the diseases thingie but on a "hardcore" mode could be kinda nice.
BUT I would hate it if we died from diseases, let's say if you contract something, you need to eat soup or whatever medicine in order to heal you but if you don't eat to heal yourself, you cure yourself automatically in 4 days.
and also, if you contract something, you cannot contract again in 3 weeks
that way, it's negative but not too much so it's acceptable as a challenge
