There's this great mod called Enviromine. It adds thirst, heat, breath in caves, undrinkable water, sanity (grow flowers to counteract going insane), and cold. And it's all in one small, easy to install package.

I know most of us want some Realism provided in Minecraft. Well, when Minecraft 2.0 Version comes out, I hope they add my Idea! Basically, instead of the 3 Gamemodes, (Survival, Creative, & Hardcore) a new Gamemode is added, called "Realistic" . In this gamemode, you have Thirst, Temperature, and an new Mob System attached to the Player. In the "Realistic" Gamemode Option, In-Game, there is only 1 Difficulty, which is "Survivor" . No Mobs will Spawn except for Animals. Monsters Spawning will be a 'Option' to those who want more of a Challenge. Except as part of realism, getting Bit by Zombies gives you Poison and in a 50% Chance, the poison will kill you and turn you Into a Zombie! There should be a way to stop this. The Gamemode will be Similar to 'Adventure' Mode where you must find a certain tool to break a block.
For the Player, there will be many different Options to the Player.
Thirst & Temperature will be explained in another heading so I'm going to explainBuilding Disabilities.
The Building System will be totally Revamped in this Gamemode. In-Order to Gather Wood, you Must Get a Rock, (Scattered on the Ground), which you can Optionally Pick Up using Right-Click on the Item on the Ground, (New Feature to Prevent Inventory Clog), and break the Tree with a Rock to gather wood, and everything Vanilla with Wood Crafting goes on from There. Same goes for Grass, Stone, and Ores.
For Weight, the more Tools/Blocks/Items you carry, the slower you get. Which provides for storing items.
Also, the Inventory is Changed. Only 2 Hotbar Slots, and a Chest slot which provides like a Backpack! Only 18 items?
For an Advanced Survival System, Thirst is added. You can Only Drink from VillageCauldrons, and Wells. But there is a more better way of Drinking. Craft a Campfire using 3 Sticks, 1 Cobblestone, and 1 Coal, and Craft a Pot using 5 Pieces of Cobblestone in a form of a bottle, and gather Water, and Boil it. After 2 Minutes, the Water is Clean to drink. You can let it cool down for 5 Minutes, or drink it straight away. Drinking straight away raises your Body Temp. to 80F, which makes you Sweat. Drinking it after letting it cool doesn't affect your Body Temp. If you End Up drinking Non-Boiled water from ponds, lakes, or the Ocean, you have a 75% Chance to gain Water Disease which kills you in 3 Minecraft Days. (60 Minutes) . If you don't drink at all and your Thirst Bar runs out, you gain Dehydrationwhich kills you in 1 Minecraft Day, (20 Minutes) . Can you stop yourself from dying? And 1 day seems kind of fast.
Since this is Realistic, you have Temperature. Instead of a long Explanation, I made a little Graph for the Different Effects:
100 - Stroke (Leads to Death) -Stay in Desert/Heated Area too long
80 - Start Sweating -Drink Hot Water
60 - Normal
50 - Below Average -Drink Cold Water
30 - Cold -Stay in Water too Long
10 - Freezing -Stay in Elevated Height, or Snow
0 - Frostbite -Stay in Ice/Snow or Rain combined for too long
-5 - Death -Frostbite
When you Catch Frostbite, or Fall from 10 Blocks or more, you might need some medical Stuff! To Prevent Frostbite from Killing you, Shift + Right-Click with a Sword/Axe of any Type to cut off Arm/Leg. When you cut off Arm/Leg you gain Bleeding, (Depletes Half-a-Heart every 2 Seconds), Slowness 2, and Fatigue 3. When you cut off your arm, you drop Everything in your Hands, when you cut off your Legs, you drop EVERYTHING. When you cut off all two legs, and two arms, you die slowly. In-Order to Stop Bleeding, Place a Block of Wool over the Leg Armor Slot or Chestpiece Armor Slot Depending on what you cut off. You can still wear Armor. To Regain a Arm/Leg, you must find a Village and trade 1 Iron Ingot for a Prosthetic Leg/Arm, or make one out of 2 Iron Ingots, 2 Pieces of Stone, 3 Stone Slabs, and 1 Redstone. When you Break a Leg, from jumping 5 or more Blocks off, you break your legs. You gain Slowness 4, and Fatigue 2. When you Break your Arm from Mining too many blocks too fast, you gain Fatigue 4. To fix your broken arm/leg, craft a splint from 2 wood logs, and 1 string. How can you get to a village if you don't have legs? And how can you mine for the items with out arms? Plus, breaking your arm because you mined too fast is OP (think about diamond picks and netherrack). In addition, do young children really need to see their own arm cut off then their character die slowly?
When your Under Ground for too long, or in a Tall Mountain, your bubble bars appear and start slowly depleting like your underwater. When they run out, you die instantly. This is pointless. If you only have, what, ten bubbles, this is too short of a time to even think of getting diamonds. Also, for a Long Period of time, Weather like Rain, Thunder, Sleet, Hail, and Snow come down in certain biomes. Also, where the storm is, There is a giant gray cloud where it's raining, and sunny where it isn't, for instense realism.
My idea is when your hardcore you can go to world type and choose Customize then you can picked to turn your Hydration/Thirst,Weight,Temperature,Climate (I don't like the cutting yourself thing) on. In default there turned off. I don't like your Idea of having no mobs,its part of the challenge on how to survive.
Eh, to be honest, I'd love this gamemode, but it doesn't seem like it'd be an actual thing in Minecraft. Plus, Minecraft's not realistic, Mojang won't want to make it realistic either, there's creepers and magic, it just wouldn't fit in.

Why does everyone assume that "realistic" zombies are infectious features that can slay you with a touch? I think "realistic" zombies could easily be slain by punching them 20ish times. And they are edible. Yummy. Rotten flesh for the win! Tasty.
Ok, now every 4 minutes? No. Your breath would be running out when you start touching bedrock
You can go into a cave in the real world and stay there all day. Cave explorers do it on a regular basis, and a lot deeper than 60 meters deep, too. That wouldn't be realism in Minecraft; that would just be added complications for no good reason.
