I don't think it would be hard at all to program into the game. It pretty much just fits around what's already there, and only requires very little art.
That's true, but when thinking of this idea I was thinking more in terms of fun than realism. 21 game days would not be challenging at all, and so it would not be fun. For a hunger game mechanic to add anything worthwhile to the game, there needs to be a challenge.
Good point, minecraft humans aren't really real humans on earth. So maybe 2 weeks? 14 minutes? Something around that... I really suck at math.
I like it, but maybe instead of the symbol changing, it would fade in from transparent to green as you start the game, then slowly fade from green to yellow, and then to red, so you could judge how hungry you are a little better.
As well as that, different foods should effect how much the colors revert, maybe the amount of hunger restored would be similar to how much health it restores (i.e. cooked meat would make you much less hungry than a raw fish)

Both really good ideas, I definitely like the fading one. But I think that the icon should not even be visible until you start to be hungry, and then the fading should happen from then.
I also like your second idea. Rarer foods should make you less hungry, as well as foods that require effort to make (mushroom stew)

Food poisoning sounds interesting, but doesn't really have much to do with the hunger system. Food poisoning is something that could be implemented even without a hunger system, perhaps as a random chance of damage instead of healing when eating uncooked food, or as damage over time which can be cured with some kind of item, perhaps a plant like you said.

He could maybe do another large update much like the Halloween update, adding a whole load of things (including this) at one time. Honestly I think this would be very easy to add. It's about as small of an update as when Notch added sneaking.

A great idea, I just hope that in peaceful mode it would be automatically disabled. But I really like the idea for real hardcore RP servers, it might add that slight further grasp of immersion. : 3
