I searched for both "maggot" and "nether worm" without real results. I also searched worms, but (of course) got a bunch of giant sand worm suggestions. What I am suggesting is different. The Maggot.
The Nether isn't very hellish. This is more of a "environment" suggestion then a "challenging mob" suggestion. Not that you should bump into them, as they still hurt (look at the teeth, man!), but they generally aren't hostile. I'll link to my famine thread if you guys want to read a "scary, hostile, nether monster" suggestion. Also, bear in mind that in the nether, where everything is red, these guys will look whitish.
Alright, let's get started.
What are their spawn mechanics?
They cannot spawn in the well lit areas. They cannot spawn on non-hell materials. They spawn anywhere that there's at least just enough room to fit them.
What's their size?
They are generally 2-3 blocks long. In rare occurances, they are only 1 by 1 blocks in size. In extremely rare circumsances, they can be up to 10 blocks long. Their head size is always proportionate to how big they are. When 3 blocks or longer, they gain additional "body" lengths. The next body length is always slightly smaller than the preceding length.
Additional note. Another common maggot size is extremely small. They don't follow the rules of normal maggots in that they don't have a collision model (just whack the block they are on), and in almost every way are more of an effect. They can spawn on the surface, which is uncommon (would be weird to see them on grass, or anywhere for that matter. Maybe spawn more in dungeons?), but they mainly spawn wherever they can in the nether. They occasionally fall off of pigmen too (spawn on the ground its walking on, less you want pigmen to be crawling with maggots, in which case I could add it to this post) .
Damage mechanics?
Touching their head length hurts you in proportion to how big they are. Giant ones ignore armor (they are ten blocks long and at least 5 blocks high, armor doesn't help much with that), allowing them to eat you whole, in a way. Its not like they can sneak up on you (I'll get to sounds next section), so it doesn't really matter that much. Giant worms are aggresive, though they are slower than you are.
Extremely small maggots will attempt to attach themselves to you when you stand on them. After a few seconds of standing on them, they will latch on, and start slowly eating away at your health bar. Every minute, you lose half a heart. There isn't a limit of how many maggots can latch onto you, and their damage effects stack, so they can be quite of a problem. To get them off, simply tap sneak, or repeatedly jump.
What do they sound like?
Kind of like dragging sounds. Their basic movement sounds are like what you would expect if you see a giant worm.
What seperates them from the pigs?
For one, they do damage. They also are rarer. But here's the main difference:
Giant maggots can break through blocks (hardly original, but better then them jumping, and is fitting for worms). The smaller worms also can put holes in walls (only bloody cobble), but they prefer normal ground (they aren't tunnel worms, look up "moles" suggestion for those). Which leads to another special purpose (for the laughs) covered in part two of this.
What seperates them from the pigs? PART 2!
If you can get to the top of a giant maggot, you can put a saddle on it (oh hey, another use for those things!), and ride it. You can barely control their direction, which allows for basic travel (Dune reference ftw? Even though these are maggots?) If the thing runs into a wall though, you will be punted off with 2 hearts damage, plus whatever damage you recieve from the fall. Giant maggots can travel through 1 block high lava pools unscathed.
Bear in mind when reading this part that giant maggots are very rare. This isn't dune, it just is funny to have occasional giants. They are meant to be 2 block long things crawling on the surface of the nether, to add to the hellish feel.
Another special feature of the maggot is that any sized one puts out fire that they worm their way over. This not only acts as lag support if your pc can't handle 100's of fire animations going at once, but also makes more space that worms can spawn in.
That was a long, two part section. What do they drop?
Maggots drop "Worm Bits". When using the fishing rod, it takes any available worm bits and uses it as "bait". When you have bait on your fishing rod, the sprite looks a little different, and normal fish come much faster. It also allows for fishing for rarer fish, which'd give more health when eaten,and/or potentially a special boost.

What they could of dropped; other ideas for drops that didn't end up in.
I ran the poll. Worm bits came out in the lead. Second was lightstone, third was acid, and in fourth place was worm meat. These were the losing options (when the poll was up):
Acid: Harvested stomach acid from the maggots. Can be put on arrows for damage boost, and maybe an added damage over time effect. Also can be used in any other recipe that may need acid, if there are any in the future.

Lightstone dust: Due to the fact that the worms will eat anything, they've gotten at lightstone dust deposits. Would make lightstone renewable. (suggested by Azraile)

Why im bumping this up:
Thought of resolution for small maggots going up a single block, because jumping maggots is the lame. How about, their head pops up to the top of the next block, and as they go inwards, the rest of their body follows. That way, it could look like they are crawling up the block.

From what you've shown these guys will likely work off of the pre-existing Slime mechanics but with more added to them.
Yea. It just has a wider size range, though its limited to a low average size with a roll that determines whether or not it can be a "giant" maggot (in that it passes 3 blocks in length). Also increases the number body lengths by twice(ish) its head size.

Nice idea and good representation! I think it would be interesting if zombie pigmen rode on some of the tops of the medium sized ones.... But another thing I just thought about, could the small maggots fit into a single block space? If so, if you could herd these corectly, they would make great living traps......

I love it! Although if they are going to be crawling up 1-block high hills, they should probably be kinda long. Either that, or they could extend like a worm. Even though these aren't technically worms, that could work. Still, it's your awesome creation.
