I realize this is a bit late, but I was looking for something like this too and couldn't fine one. So created one myself, but not sure if this is the best way or not. Maybe someone can find a more simple machine to do this. Might be overthinking this.
When you turn the lever On or Off, a signal will go to the note block and it will play a tune.
The repeater in front of the torch has to be on state 0, and the one beside the torch has to be on state 1.

I think this is what you need..
You could actually get rid of the redstone dust and everything.
Only thing you need is an observer coming from the initial signal, facing into the output.
EDIT: You will receive a 1-tick pulse doing it this way, but you can delay it with either a repeater after the observer or a monostable circuit.
Yeah...I realized 5 mins after posting that there is an observer now. Guess if you don't have the nether yet or wanna do it the old fashion way, you have the other method too.

I need to pulse whenever my initial signal either turns off or on. Doesn't matter what state, I just want it to detect when it changes.
