"Duration" and "Amplifier" can be whatever number you choose. "false" simply means there will be no particle effects. If you want particles then simply change it to "true".

So, those are the three commands. Hook them all up to a fast clock in the order I posted them. If in 1.8 that would be a /fill clock. If you are in the latest snapshot, Put the first one in a repeating command block, facing into a chain command block with the second command in it, and face that command block into another chain command block with the third command in it.
And there you go:D! If it's still no working, go back and make sure you spelled everything correctly. (It is important that you get letter cases and spelling correct). And if all that is right and its still not working, try using an anvil to rename the head to whatever. ("Dinnerbone's Skull" for example). Them make sure you update the command block to have the same name inside it and try again.

Firstly, you have to create a scoreboard. We'll call it, WeakSkull.
Okay, now that's out of the way. Next thing, We have to hook the next three commands up to a fast clock.
