What is your video hardware and video driver version? If you're using mods (OptiFine is included here) then list them, as well as if you're using certain settings, for example if you're using multi-core chunk rendering/loading (I have no idea if this setting was available for OptiFine that far back though).
Some old versions, such as 1.0 through 1.7, do have known rendering issues these days. This doesn't appear to be one of the them though. It looks more like what would happen if either the video hardware (or at least its memory) was unstable or failing, or if the game was trying to do something the video drivers don't like (or running our of memory/video memory). If the game crashed afterwards, it could suggest either.
