I use torches because I do a lot of mining and caving and it's so convenient to not have to switch what I'm holding.
And enchanted diamond armour is so OP that I very seldom need more protection.
I really ought to use a shield when I start a new world, before I get fully enchanted armour, but to be honest I tend to forget that they're a part of the game.

You only give a choice between a Shield and a Totum of Undying.
Average players in average circumstances should find a shield to be quite sufficient for normal adventuring.
If you live on the edge, constantly in danger of perishing, a Totum of Undying might be the right choice. You should have a lot of them though, because you might waste them unnecessarily by mistake or in moments of panic. I have never even had a Totum of Undying, but when I do get one, it will be stuck in an item frame and put on display.

You don't use Totems of Undying, they are used automatically if you are holding them when you would have died.
Very rarely do I have a shield in my offhand and I never have a Totem of Undying (maybe it would be if I had ever found one but not yet so far). Generally it depends on what I am doing:
Building: Either the same block I am currently placing (so I can put down 128 without having to change slots) or if I am placing 2 or more blocks frequently whatever I am using most of so I can quick swap between them.
Gaining XP in Large Amounts (doing a high number of trades, emptying furnaces, breeding/killing my penned passive mobs): Any tools that have Mending and need repairs go in my main and offhand slots for these actions. If nothing needs repairs then it doesn't matter.
Combat: If don't have decent gear yet, then I might carry a shield, bow, or food. Once I get unenchanted Diamond or better then it is either a bow or food, because I can then kill anything before it gets close or just tank/heal through the damage
Underwater Combat: Food. I can outswim a Drowned's ability to throw tridents 95% of the time, and if I get hit my armor lessens the damage to where I can heal through after killing it.
Mining/Caving: Torches

Nothing, as the offhand doesn't exist in 1.6.4 and even when I played on 1.9+ a bit just to compare the experience to 1.6.4 and see how effective Mending was I never thought of using it, same for double-tapping W instead of using Ctrl to sprint (which to me means zoom, either with Optifine or my own mod which adds a similar function).
More specifically, as far as shields go, I'd rather rely on general armor protection, which has to be enough to reduce a point-blank creeper explosion down to no more than half your health, than something which must be used and is highly directional, and I practically never die so a totem wouldn't be that useful, and in the sort of situations where I might die I'd probably be killed shortly after it activates.
Whilst I have a few Totems of undying from succesfully winning raids, In general I have a shield as default - even when I'm not playing my main survival world of 9 years+. It's force of habit. Skeletons, pillagers... I just prefer having one. Pre armor, in the early stages of a new world I might have an altrernate pick axe, one of the better ones for ores, I can just switch out too.

personaly i use neither of these unless i need to fight the dragon or doing end exploring i always have golden carrots on my offhand
