I've played on a total of 8 "main" worlds, meaning worlds I exclusively played on for at least a few weeks, and 5 "secondary" worlds, including a custom map (Stratosphere Survival, which I played when AKSpartanKiIIer played it) and Survival Island (seed "Notchiswithus", Large Biomes, in 1.5.1/2. These were both before I became obsessed with caving).
I've also made hundreds of "test" worlds in MCP; currently I have 28 worlds I made for testing various changes to the next version of TMCW with names like "CaveTest(number)" for testing various changes, although those don't really count since I don't actually play on most of them.
That said, I currently only have two "real" worlds - I don't keep worlds I no longer play on, and only one of those two worlds is currently active (I have not updated my signature since I stopped playing on it). I have kept various renderings and screenshots of my "main" worlds though, most of them also have various threads and posts for them:
World1 (my first world, and only one that I've kept and returned to over the years, between time spent playing on other worlds. Also the only world which was created in vanilla, and by far my longest-played world with more than 109 days of playtime; it took nearly that long to find a Mushroom Island, the only time I've ever found one in any world, as all of my exploration is underground):
World1v2 (the first world I modified cave generation in; i just increased the size and decreased the chance, leading to more varied and concentrated cave systems, pretty much the opposite of what 1.7 did. I also tried making mountains higher, although at the time did not know how to remove the 128 height limit):
World1v3 (only caves are shown; all the worlds so far have used the same seed. This world had "double height" terrain, or more accurately, double the ground depth, which was increased from 63 to 127 (sea level) and caves and other underground features scaled up to match; unlike the increase in terrain height in 1.7 or in my later mods this simply raised terrain by that amount):
TripleHeightTerrain (similar to the world above but ground level is now 3 times deeper with even larger cave systems, some 30 times the volume of vanilla. After this point I decided that the vanilla ground depth was best and now I only remove most bedrock and shift everything down by 7 layers so there are 59 instead of 52 layers between lava and sea levels):
TheMasterCaversWorld (the first world I created with the mod of the same name, which was not actually named until later on):
TheMasterCaversWorldv2 (created with the second version of TMCW, which added more biomes and more varied caves):
TMCWv3 (using the third version of TMCW, with more of the same, plus terrain able to exceed y=128):

I had a hard drive crash in 2012 and bought a new computer at that time so my worlds only go back to that time but I do have a few Beta worlds.
I have separate game directories for each version of Minecraft that I have profiles for.
So I have separate save folders also.
All in all I've got about 460 world save folders.
I only actually play about 3 or 4 of them with any regularity.

As you can see, most of my worlds are for testing mods, command blocks, redstone, etc. Most of my "Custom Map Projects" were adventure maps I wanted to make, but didn't put a lot of effort into. The only four "Downloaded Maps" were meant for my Vanilla install of Minecraft, considering I can't play with a lot of popular modpacks with my potato PC.
EDIT: And this is only counting the worlds on my personal laptop. I'm sure I've left more on other computers in my house.

And I have two Java MC directories that are active, and something like 28+ inactive worlds in backup folders, some of which may be duplicates of each other, or of active worlds. I think I have backups of some W10 worlds, but I'm not sure where I put them.
In my current most-used Java profile, I have 8 worlds, three of which are purely test worlds, one that started as a test but I play in a bit, one straight survival world, one hardcore survival world I currently play, and two hardcore worlds I kept around to examine in spectator mode.
So, I have anywhere from 2 to 40 worlds, maybe? Not counting stuff I just plain deleted?
My count is constantly changing. I have one survival and two adventure maps that I'm making right now, and I also have a few downloaded adventure maps that'll be deleted upon completion.

I have 2 from 1.2.5 with mods that do not have any new content added, 2 other 1.2.5 worlds, one with the mods from Direwolf20's season 4 LP series, another with 1.2.5 that has most of those mods (I removed Thaumcraft 2 from it and added Chocolate Quest (called Better Dungeons then, with a companion mod called Adventure Items), then the other 4 are all using FTB Direwolf20 modpacks, 2 1.4.7, a 1.6.4, and the one I am currently playing, 1.7.10.
One of the 1.2.5 worlds is the original one I started playing Minecraft on, way back around Alpha 1.22. The other of the first two was started when Minecraft 1.o was released.

The reason I have so many worlds is (for the most part) because I like to give each structure it's own world, otherwise I end up with worlds full of random unrelated buildings. Also my old computer couldn't handle too much stuff in one world.
The last time I checked, around 10. Most of them were world I started but never finished or got far. Because the humble beginnings and simple livings of the early first few days is my favorite part of the game. I love it when things are intentionally simple and living is easy, and there aren't any things like diamond tools and armor to distract from this living.
But that was more than a year ago on my old computer, so the answer is actually zero.

Realistically, just one, and that's all I've really ever played. The first world I started in 2012 has been the only one I've consistently played. So, just one world for me.
Literally, I have something like four or so at this exact moment. One is the main survival world, another is one I generated with the same seed just to explore and see how much has changed, a third is the obligatory creative world for testing builds, and the last is a second survival world (there's been probably about half a dozen "second survival world" throughout my time playing and usually there never ends up being a third, if that hints at it's possible fate one day). Teeeeechnically... there's like two more counting worlds I've started just to play LAN when I don't have access to this PC/world, but they were more or less one or two time plays that probably don't have futures either.
Short answer, one survival world and one creative world. I don't intend to ever start and keep a lot of survival worlds, but I wouldn't like to start that second one and stay with it, but have trouble doing so.
I probably have at least 50.

I have about 25 about 50% of them are just for mod testing and the other 25% is for my survival and the 25% is for my buildings
Huh, more than I thought - 13, although only five were ever "real" worlds, and only one is currently active.
