Some people would like to argue that it'd be dirt, but it has some uses. Filling in ugly creeper holes, providing temporary protection at a moment's notice, filling in those spots where you mined ores, etc etc.
So what do you think might be the most worthless item?

Really? I've bought around a double chest of diamond pickaxes from villagers in recent months, ever since I found a village with a blacksmith selling them for the low cost of just 11 emeralds (unenchanted, as I'm in 1.6.4, but that just makes things easier, as opposed to possibly getting Fortune or Silk Touch since I use them to repair an Efficiency V, Unbreaking III pickaxe). You can also buy other diamond gear (prior to 1.8, all pieces), and in 1.9, the most valuable enchantment of all, Mending, which can only be obtained by trading or found in chests (this lets you have infinite gear for the cost of only XP, you don't even need to mine more resources or use anvils to repair them; 1.8 is the only version where you can't keep the same items indefinitely).
I'm surprised nobody mentioned poisonous potatoes; while you can eat them they can poison you (poison, not hunger, making them worse than rotten flesh) and they have no other use.
Ender crystals. They're worse than useless in the End, since you have to shoot all of them before you can fight the enderdragon.
No they are adding in a way to bring back that big dragon. And guess what. You need the ender crystals to bring him back.And you can get these crystals from skeleton horses.
My nomination is the Dragon Egg. A specific End update later, and it still does not really have a true use. If it were required in addition to the Ender Crystals to summon the Ender Dragon again, I would find that realistic.
I was just about to post this, the dragon egg has literally no use. You could display it and use it as some sort of decoration, if you're careful that no one touches it so it doesn't teleport off somewhere. Apart from decoration it is completely useless.
I'm surprised no one has mentioned leather and chain armor. It's typically much harder to gather resources for a set of leather armor than it is to find enough iron for full armor and leathers low level of protection means it's practically useless. Chain armor is even worse since it can't be crafted and is only slightly better than leather. There really is no point to either of these armors seeing how plentiful iron is. I often wonder if anyone really uses them outside of themed or minigame servers.

I think it might be dead shrubs. They serve no purpose except to add a bit of pizzazz to the desert.
Insulting people for their beliefs is not a good way of convincing them to adopt yours.

As it isn't released yet, I didn't take it into account.
Especially for fire places, there the only material that let's fire burn forever.

Actually, all the blocks in this game are useless IRL... they're all just pixels, right?
In 1.9, you can get sticks out of them, so not completely useless once that update comes out.

The most worthless item in game I'd say wood be wood tools except for the pickaxe and as soon as I mined some stone I stop using that.
