Also, another thing that drives me nuts- when they dun't put down torches. You're in a cave! Light it up! Then, when they run through an explored section of the cave, they start ranting when a creeper blows them up. You were there before! If you lit it up then, you wouldn't be in ten dozen pieces!
No, no, no! Both of you have got it wrong! When mining, it's ore 1, ore 2, shovel, torch, pick, sword, food, ore 3, ore 4. This way, you can access your sword and torches easily while mining. Shovel and food are nearby, while you can keep track of your loot.
Basically, I find myself thinking this while watching- "No! Torches go in slot nine, not six! Ugh, food belongs in either seven or eight, yet you have it in four! And the tools are all wrong! It's Sword, Bow, pick, axe, then spade, not spade, sword, axe, pick, bow!"
It drives me insane.

In pvp servers when people cut down the bottom of a tree but leaves the top portion they can't reach...it looks horrible yet people are so willy nilly about it!

Not completely cutting down trees.
Not replanting trees.
Horribly defacing the surface.
Mining stone with iron/gold/diamond(Lets face it, unless you have an iron generator, that use of iron pickaxes is a waste of iron).
Wasting food.
Mining in caves.
Not using lights.
Not anti-mob-spawning your house.
Improper crop placement.
Ect.
I hate it when people mine cobble and ores and stuff with their hands, when they have tools. I mean, even if they didn't need it, you have a pick right there!
When they have a couple random mob drops in their inventory
Except I have a really bad habit of getting rid of random mob drops like that whenever I kill something and it leaves me with nothing
When people go in caves with 10 torches, and just like 10 wooden planks/sticks to refuel them, and then complain about running out of torches.
Also, when their hotbar isn't: Crafting Table, Furnace, Pick, Sword, Shovel, Axe, Misc., food, torch.

It's funny how you guys think that it's annoying when people don't set up their hotbars exactly like you. I don't find that as annoying because it's just preference.
I have multiple pet peeves: When people have unorganized inventories and chests, when someone uses a weird texture pack, when they use the wrong tool, when someone does something stupid/dangerous, when a door is not in the middle or facing the wrong way, when someone's hotbar is messy, when they don't know what they're doing, and I could go on, but I'll just stop here.

When people don't make even-numbered buildings. By that, I mean that they make houses/buildings/farms that aren't an even amount of blocks across, like 6x6 or 4x4.
I'm actually the exact opposite. I like my rooms 7x7, 9x9, etc. My brother and both of my sisters and I all have our own Minecraft accounts, and we like to play LAN. One time I making an underground house for my sister and I, and my brother comes down and starts digging some out. I kind of had a bit of OCD when it came to that, and I explained to them why I wanted to make it certain dimensions, and they just laughed at me for being OCD about it. My siblings are jerks.

Actually I usually mine with an iron pick. I usually get more than I use, so it's worth it. Diamond on the other hand is a different story.
PS. The correct inventory organizing, from left to right, is:
Pickaxe, Axe, Shovel, Sword, Food, Food, Block, Block, Torches.
