Topic says it all.
Post your biggest fail you ever made.
Here's mine:
I was mining pretty deep down, a few blocks higher than bedrock, and my pick broke. Didn't have any coal to smelt my iron, and only had two diamonds. So I went looking for coal to make a iron pick.
A few minutes later, I found a huge vein of coal. Last piece was on the floor, Stood on top of it, Mined Down, Turns out there was a lava pool under it -.-
Lost two stacks of iron ore, one emerald, a full armor set of diamonds + the other two that i mined, and 3 pieces of coal
Stacking your way up in an underground ravine. Seeing how it intersects a mineshaft and several caves, I was doomed to fall by a skeleton. In creative, a malfunctioned huge tnt cannon. Never attempting that again.
It would have to be mining glowstone on the ceiling in the nether, and getting blasted off by ghast fireballs, falling in lava and dying. To prevent that again, I build these huge cobble stone enclosed platforms around the glowstone. My nether has all these ugly cobble squares hanging from the ceiling.

My biggest fail was when I was fighting blazes. I had a full suit of diamond on me and one of those DARN ARMOR-PIERCING FIREBALLS OF DEATH hit me and I fell into a lava ocean. Lost so much for so little :l
Friend was over (PS bofore he was over I built a cool secret room) and I was showing him my house I built. As I showed him the farm outside, we came back and my lava fireplace burned the house down (also exposing the secret room but he luckily didnt notice it). I put some glass around the lava so no embers would fly out. Left. Came back. Burned down again (I was a little nooby and didnt know that any wood remotely next to lava would start on fire). He found the secret room :P, I rebuilt the house again. And it rained after my house already burned down twice
My Biggest Fail Was Also A HARDCORE NOOB MOMENT! I Was On My First Survival World, And I Was Feeling Like A Boss, And I Had Just Gone To The Nether And I Wanted To Make A Fire Wall Around My ALL WOODEN BASE! So I Lit The Netherrack And That Same Day I Died By Burning To Death. And This Was Only Last Year.
Early alpha I was mining in a cave; I had just stopped to throw away cobble, smelt down ores and throw away excess stuff. Followed the cave through a bit further and it came to a patch of bedrock and I fell into a 2 deep bedrock hole.
At first I didn't think it was a big deal and that there had to be some way out, but after about 10 minutes of looking around and jumping it dawned on me that I'd just broken my entire world. Eventually I installed and used MCEdit to move my character out of the hole.
My biggest fail in MineCraft is where I was walking around with bunches of TNT and redstone torches and I didn't know I had placed some. Then all the sudden "SMACK!", I'm dead. I lost like 20 - 25 diamonds and enchanted diamond tools and armor and I was like "...Fluffy......"
Yesterday night I had ANOTHER epic fail.
Trying to test this redstone contraption underwater. Took me 30 min to build. Made walls around it, no door.
After I finished testing it, before i could take a screenshot, i broke a block to get out of the room (broke the block from the roof).
Didn't notice and kept going. Ten minutes later, my friend logged on the server and I remembered to show him the redstone thingy.
Came back with all the redstone wiring gone due to the water

This is what happens when mechanics meet water = doom.
Anyways, I have a similar experience like you. I was building an automatic cow-killer for my cow farm (it involves crushing the cow with pistons), and I have to install this water flowing thing to collect the raw beef when the cow dies. I built a small platform above my cows, and I aim that water bucket at that particular spot, and just when I was about to click my brother exploded into my room. My hand jerked and clicked at the same time, and the water caused a gigantic waterfall that drowned all my cows and destroyed some redstone on the floor and I fell off my platform, so I couldnt stop that waterfall.

I turned around and gave my brother a bashing.
So I'm not alone
I would of killed him if I was you.
Got killed deep in a mine. Somehow managed to get back to where my stuff was in time. Had to run to a safe location so I could re-equip everything, but then I got lost and was killed again, in such a way that I would never find where my stuff was. Lost: 6 diamonds, tons of redstone, coal, emerald, lapis lazuli, gold; in short everything.
But it was all right! I could still get stuff in that cave system, it was enormous! So I went back there, and all but cleaned out what was left, including some more diamonds and tons of everything else, then magma got me. Plus I was too far down and never would've gotten there in 5 minutes even if I knew where it was.
This happened when I was still a semi noob with a Steve skin.
I made a 3x3 lava incineration pit for all that useless cobble i got from mining. I was wearing full iron armor with a stack of iron blocks, 10 emeralds, a jukebox disk, a stack of red stone, and some unplanted melon seeds. I wondered whether I could jump into the lava, jump back out and extinguish my self.

Cheats were enabled solely for teleportation needs.
I teleported into stone and died :c

I literally swam in lava.
