I think it has something to do with lag either by the computer, or the net depending if your playing online or not. I got this once in a while back in Vanilla too on both computer & 360. I also get this happen a lot on CastleminerZ with blocks. Its more annoying when you fall threw the world!

I HAVE A FIX!!!!! I had VBOs off to increase fps, but after turning them on the problem completely disappeared. Hope this helps!
