Alright, my house is situated on a two-tier cliff. When I built the house, I didn't have any coal, so it was pretty dark in there. Let's just say a creeper MAY have spawned, MAY have blown up part of my house and launched me out the window. Let's also say that I MAY have landed on the bottom tier of the mountain/cliff with a few hearts to spare. Let's also say there MAY have been a skeleton hiding under a tree on that lower tier, and it MAY have shot me off the mountain into a ravine with lava at the bottom.
An admin spawned some blazes in the oveworld. He gave me some eggs and said "you try" So, I tried to get rid of the eggs by dumping them in a lava moat, but I right-clicked instead. That was one angry blaze.

I was on a multiplayer factions server a couple weeks ago.
My entire faction decided to revolt against me because I had fully enchanted diamond armor and I "should have shared". (The armor in question I earned on my own before even starting that faction).
Ten of my faction members were online at the time and all of them geared up and trapped me in the basement of our castle. I backed up into a corner with my potions and was able to kill most of them more than once.
Then they all brought in lava buckets and flooded the basement. My health was already fully regenerated but my traitorous faction was already out looking for me again, and we were a really rich faction so most of them had one or two pieces of enchanted iron armor and an enchanted weapon.
I dug a small hole into the side of a mountain and then blocked it off so I could type the command. I did and the faction was disbanded. I dug out of the hole and then sprinted in one direction into a grove of trees.
My former faction members were waiting for me and surrounded me instantly. I killed two of them before dieing.
It was beta 1.7.3. I had a mod installed on my server, something I was trying to write to add a magic system, like necromancy. So. I pick up a stick and add an item of the mod to it, a DOOM ore (caps necessary, normal doom ore was something else, because I was 14 and stupid), and I swing that thing. 15 chunks around me get deleted, and the one I am standing in fills with creepers. I die, game lags, server crashes.
I was adventuring with a friend, and I shot at a spider, but hit one of his 4 dogs by mistake. They killed me, (Although I did put up a courageous fight) and I re-spawned at my house. Then my own dog attacked me, but I managed to beat him to death with bare fists. My friend came to give me my stuff, but two of his dogs came after me. I fled into the Nether, thinking I would be safe, but they followed me even in there! He finally got them to sit, and now they just sit there by the portal, growling at me. (We call them Cerberus and Orthus because of this) Let's hope they never stand up again.
Well, I was down in my Mine, having a normal day, in a normal life, in a normal world, when suddenly I got to a part of the cave I never had been to before. I didn't see anything (Changing Options from Dark is cheating!) so, I decided to try to find a end of cave. I managed to Walk to the end, Crashing into a wall. A wall with some wierd minerals... Hmmm... I tried my Stone pick, it didn't work. I finaly managed to see it was Diamond (I had few of them ATM), and shouted out "This is my lucky day!". It wasn't. I heard Footsteps. I turned around, crafted a torch in about 3 secs, & placing it down. Suddenly, the hole floor falls down, & I realize that the Lava sounds I heard before was here. Me, and the Creeper, burned to death in secs, but I killed him with my sword just before he died.
It was the wierdest Cave I've ever seen: After comming back, I see that the Lava pit was VERY deep, and the Gravel was Spawned in the air, and that placing the torch would make it fall. And the diamonds were the bait. What's the chance of something like this spawning in the world of blocks?
I was on a server, and a friend threw a potion at me. My sound was turned down, because I was listening to music. I thought it was a poison potion, because my health was going down little by little. I stood there to chat, my health still depleting, until I died. I was actually leaning my back on a cactus, without realizing.
I entered the Nether for the first time on the PC version, thinking I was prepared to fight blazes and wither skeletons, but I was totally wrong. I was cornered and killed by a blaze and a wither skeleton.
two guys and I were playing on a multiplayer raiding skyblock server and we found an xp grinder. so we went to check it out and when we got there it was pretty good. until a creeper came and blew up at the base and make a giant hole. next thing we knew mobs where flooding out of it like crazy so we where pinned down in this one guys house making our last stand. we where being picked off one by one then finally a spider jockey jumped the hole we made and killed me.

I made a new world and I spawned at the top of a mountain. Lag made my controls mess up. I fell off the edge at least 115 blocks. Hit a creeper with a skeleton next to it. I died from falling and the creeper. Then the skeleton shot me with 7 arrows.Then I spawned not thinking and fell off the same exact mountain onto the exact place i did last time. With the skeleton still standing there shooting my body with arrows. I got my revenge by using the nuke mod and destroying the entire world except for my berock house that I made in creative.Worst and best day of my life in Minecraft. =D
I was PVPing with a friend who had a diamond sword with Knockback 15 or something because of a plugin, and when he hit me, I flew over our PVP arena walls and into a very small lava pit... It was awesome.
The coolest death I've had?
Well, all deaths suck.
I guess I could say that it was the time I used a bed in the nether not knowing that it would blow up.
The floor blew out from under me and I fell into the lava ocean.
This happened on the Xbox edition. Our nethers are tiny, 200+ x 200+ in dimension, you can see from one end to the other if you stand in the middle of it. Before 4J made glowstone dust a farmable drop from Blazes, glowstone was painfully limited.
So a friend and I were in the nether once on a glowstone run. It was still the 1.7.3 equivalent, so no health regen or stackable food, nor sprinting. My friend was in charge of carrying resources, I came in with loads of cobble for making scaffolding. We were pretty irresponsible for making a clean sweep for glowstone in one run, but we gathered as much glowstone as we could find. We started back toward the portal, neither of us doing really well for health or food, and having already spent all our arrows against ghasts.
Still a good distance from the portal, my friend draws aggro from a ghast in a large open area, and while trying to punch out fires while running, also accidentally punched a pigman and drew aggro from a crowd of maybe a dozen or so of them. I don't know how he ended up so far behind me an on a slightly different route, but he started squealing over the mic, so I turned back, dropped the last porkchop for him, ran past him back toward where the ghast was. Right as I passed the mob of pigmen, I hit the last one in the group, turning the mob's attention to me. I ran up under the ghast, threw my remaining cobble down and pillared up to it, smacked it in the belly with my sword, and fell to my death off the platform becuase I wasn't really paying attention to my forward momentum.
So, coolest way I died: self sacrifice to ensure the return of our spoils. It was pretty cinematic how it played out.
