Every single door must be closed before I leave my house BECAUSE CREEPERS ARE EVERYWHERE O.o also chests have to be super organised (cobble in one, dirt in the other etc) and ALL my roofs must be made of glass (just for the lulz of spiders stalking you from above)
I must mine all ores. Even if i have 5 million redstone, I MUST GET THAT 5 VEIN OF REDSTONE! Even if i have 5 trillion coal, I MUST GET THAT 31 VEIN OF COAL!
I really dont care about Cobblestone. It's cheap, effective, and resists explosions to a degree. I like it, It works! But god help me, If i see an inch of cobblestone in my smoothstone home, I'll rip your eyes out, and feed you to the creepers (unless it's temporary repairs from said creepers). (i'd say something else, but i'm afraid i'd get in trouble here)
I must Fill in the holes i create if i use the space regularly. No air pockets. I KNOW the darkness is there. I know it exists. And i am afraid.
No Flint and Steel use unless i have to, but it is something i quickly craft.
I dont make Diamond armor unless i have stack upon stack of them. It's pointless, imo.

I carry around multiple stone picks. I'm talking up to 14 or them. I also carry a diamond/iron pick (whatever i have).
I Only use diamonds on Swords and Picks. Nothing else. I make the other tools out of stone. I also carry a stone sword around for animals.

1) Symmetry.
2) Every room must have dimensions in multiples of 4. 2x2 Columns are placed with 3 spaces in between them so the 4 multiple always includes one wall. In large rooms ceilings must be 7 free spaces high. Including the floor this makes 8 which fits with the 4x rule.
3) If something is built in the diagonal spacing, the opposite wall must also be diagonal.
4) Rooms must have an even number of walls. No triangles or pentagons!
5) In branch mines torches must be placed every 5th space on the block at head level.
All branch mines are built from a central hub and each tunnel must be 32 blocks long in every direction before hubs at the end of those tunnels can be created.
6) Mountain bases must be built entirely out of smooth stone and glass. Nothing else.
7) Castles and towers may be built with cobblestone but smooth stone and cobble cannot be mixed within the same structure.
8) Mountain base interior doors must be iron, pressure plates on both sides. Exterior door is double wood with pressure plate on the inside. Currently working on redstone double iron doors to use button on one side and pressure plate on the other to replace this.
9) Ocean inlets must be at least 2 blocks deep and have sand bottoms.
10) Every chest must contain one item type only, no mixing. One misc large chest is allowed. Colored wool may share a chest.
11) Water must be stationary unless its a water fall or being used for transportation/mob traps.
12) Inventory bar left to right: Sword, Pickaxe, Shovel, Dirt, Cobblestone, 3 spaces for items pertaining to current project, Torches.
13) All sheep must be killed.
14) Tree farms must have 2 empty spaces between trees in every direction and on the edges must be at least 2 spaces from a wall.
15) I must be able to walk under every tree, no leaves within those 2 spaces unless its natural and outside my current living/project environment.
16) No surface transportation systems. Underground only. Skybridges are allowed only between mountains that are already close together or buildings.

I always have torches every other space for smooth, consistent lighting.
My hallways are always 3,5,7 wide.
My mine entrance is 3 wide, made with cobble stairs(custom texture pack makes it look like tile flooring)Every visible wall/ceiling tile is smooth stone.
All doorways/entranceways in mines have a cobble 'door frame'
My storage room is... well... I would probably be worried to show it off, lets just say that I have 2 double chests for cobble... and glass... and diamonds... and cooked pork(bacon!)
Talk about shock absorbency!(or OCD >_>) I have an entire mod that organizes everything into neat little stacks and sorts them by item ID, and makes as few stacks as possible.
And I tend to keep like things together in chests, if I have enough like things.

1. Gravel in my branch mines: I will not only cover up what is currently at floor level with cobblestones, but will go down as many levels as it extends in the corridor I'm mining. I usually leave the gravel in the walls alone, unless there is ore under it or nearby. Then the same applies with filling in what has fallen down.
2. Creepers: If I see one, I will shoot it on sight, even if it is on the ground and I'm safely above in my glass enclosed minecart tracks.
3. Torches: I put them at regular intervals indoors and outdoors in areas I am trying to light up to prevent aggressive mobs from spawning. In my branch mines, I space them 10 blocks apart in the main tunnels, and 8 blocks apart in the two cross tunnels at each end that intersect with the main tunnels.
4. Trees: I replant everything I harvest, often manually clearing away leaves. Bonemeal makes it easy to regrow them.
5. Minecart tracks above ground: I always elevate these and enclose them in glass, with torches every 8 blocks.
6. Ore: I will mine out everything I see, though I will sometimes ignore coal and redstone unless it is in my way. If I need some of the former, I'll go back to veins I've missed and mine it out. I fill every area I've mined in with cobblestones.
7. Doors: I wouldn't call this so much OCD behavior, more of a CYA to make sure mobs cannot enter your house while you're away from it. I learned this once the hard way when I left my front door open one morning only to find a skeleton inside when I returned.
I have CDO, which is like OCD, except the letters are in alphabetical order, AS THEY SHOULD BE.
... but regarding the OP, the most CDO thing I've ever done was stand in one place, adding a block, then immediately removing it because I didn't like the way it looked. Then I'd remember what I was doing in the first place, and replace the block, just before immediately removing it again. I think the longest I've looped like that was two hours.
Close and open the door three times per entry/exit even if it means death
