I always (ALWAYS) make every building/cave deliberately complex/confusing/dangerous for PC's (even on single player) and NPCs.
I've gone so far as to make an obsidian trap that I can seal off a tunnel with, lava doors that have the minecarts on odd angles (I died a lot making these) and putting the my golden apple in a chest near a mob spawner just in case some gets into my single player game and tries to steal it.
- I always level out an area before building.
- I have too much security. I have a treasury where I keep my valuables, and the walls are like this:
All over the place. I'm paranoid about creepers.
- I have to have a main power source that is linked up to any power source with an AND gate. If the main power source isn't on, no redstone works in my city.
- Doors MUST be closed.
- My automatic boat dispenser and cart dispenser MUST be restocked at all times.
- Whenever I take my unicorn or my pegasus (Mo Creatures) out for a ride, I seal up my cage when we leave, and open it and seal it again when we get back.
- I always have an underground escape route leading to a nuclear bunker with 3 block thick walls.
Lately, I've come to realize I have a habit of placing torches on the top of blocks, rather than hanging them on the sides. In fact, for some inexplicable reason, I get really annoyed if I have to place them on the sides. So much so, that I will mine and reshape the blocks in the area so that I can place the torch on the top of the block.
I have no idea where this habit came from, or why I have it. I just do. I guess I think torches look better standing straight up, and perhaps their lighting is a bit better too. It's probably all just in my head though.
When farming lumber, I always replant a sapling in the exact spot as the original tree. When I chop down trees, I have to collect every single piece of stump that falls. This includes abnormally tall trees and gitant trees with branches.
When mining/cave exploring, I always have to clear out every single piece of drt/gravel in my way, and all visible ores must be collected, and then that part of the cave/shaft/tunnel must be repaired with either stone or cobble to look nice - no jagged edges in the corners or strange shapes. When I hit an ore vein, I have to collect all of the ore blocks in it before moving on.
I always build a shelter on my spawn point in SSP, and my spawn coordinates must be in the exact center of the main room of the building.
Torches must be spaced out evenly.
I hoard everything, even dirt. It helps for when I have to clean up creeper explosions or do other landscape fixes. I can't just leave the landscape messed up after an explosion or other other alteration.
I label all of my chests with signs, and place one type of material/block per chest.
i have to make sure all creeper holes NEVER get filled. infact sometimes i just like to get some and blow **** up and just bask in the glory of the total chaos that is my mine craft.
another OCD thing i got is that i like to **** the **** out of every passive mob i come across. if its mental state is intact than i just dont feel right.

Another OCD thing i got is that i like to shoot the ****ing **** out of every passive mob i come across with a flamethrower. If its body state is not a pile of ashes then I just dont feel right.
-Make my houses Odd Dimensions (5x5x5, 3x3x3) so I can place torches that are one apart from each other. Two torches next to each other looks ugly.
-When digging to bedrock, I sneak on a block and look at the block in front of me, I dig ONLY 3 blocks down, when you dig 3 down and jump down...you take no damage, then I dig out a ledge for me to stand on (2 blocks high) and repeat.
-Whenever I find a large vein, after clearing it out, I always fill it back up with stone (NOT COBBLE STONE BUT SMOOTH STONE)
-Whenever I find diamonds, I horde them and never make anything out of them
I hack ice blocks into my inventory to fill the holes creepers and misplaced torches make in my frozen lake. This is practically the only time I use invedit.
If I encounter dirt/gravel in one of my mines, I must clear it all. Oddly, I usually don't fix the ugly holes it leaves behind.
If I enter a cave, I can't leave until it's 100% lit up.

i dont carry a hoe on me ever, who randomly decides to farm when theyre exploring?? i just go outside and rake up a huge section with a diamond hoe if im farming and really need seeds(only needed to do it once to start my wheat farm, my hoe still has half uses and i have like 12 stacks of wheat and 5 stacks of seeds ill never use already).

Seriously though, on my current world there are a few chunky patches of sand scattered around the area I'm building on. My project won't be complete until I dig every single one of them up and replace them with dirt. Also if I'm walking along and I see a random dirt block in sand or vice versa I tend to rip it out.
I'm not sure if this is so much OCD as not wanting to get lost, but I feel compelled to cover up any cleaned-out (no more useful resources in it) passageway that leads to a dead-end or loops back in on itself. I don't get too hung up on the material I use (though I used to only use smooth stone), but it has to be covered up well enough that I can't see it anymore (or else I'll accidentally dig it out and explore it again). Despite that, I won't cover up passageways that only have coal in them, even if I have the resources for enough torches to light several cities.
When I mine glowstone, I always make a large square platform of dirt to catch the stuff, even if it'd be more efficient to cut corners and make a more circular or oval-shaped platform.
The only symmetry I use is 2x2, 2x4, 1x2. I cannot ever have 3x3, 4x3, or walls equidistant to me for longer than two blocks.
There must never be a revealed torch.
Symmetrical lighting effects are fixed immediately before anything, mainly floating islands. Spaces that look like rooms in caves are removed completely so the whole surrounding area is open.
Never similar objects stacked against each other, except for stoves.
The ceiling can never be three high.
I never waste fuel, ever. I use less than 5% of my iron and less than 1 in 50 diamonds. I store every object. I fix chunk errors within far distance range of my build. Collect every sapling, never deform the terrain without a good reason, I'll travel 45 minutes to get sand.
Every sheep.
Never use red brick, cobblestone, paintings.
Nothing I build can ever attract the eye towards the center of two objects (walls, mountains, etc).
I use hexadecimal for everything.
