i MUST pick up every block i destroy. creeper holes must be fixed (eventually). i cant stand random holes that lead to caves, fill em up. if i'm planning a site for building, the area must be flattened around it.
but what im most OCD about is the spacing of torches and the ordering of my hotbar.
1: pickaxe
2: sword
3: shovel
4: axe
5-7: random stuff i use then and there (like dirt and such)
8: watch
9: torches
im so used to this setup that if i change it i end up trying to break smoothstone with an axe for example.
i always put extra pickaxes over the hotbar-slotted pickaxe. likewise with all other tools. the 3 slots over the ninth slot is reserved for extra torches, ladders and/or food.
caves MUST be lit. even if it is only the roof and there is no chance of mobs spawning there, i put a torch up there.
that's pretty much my OCD's for minecraft

I can't stand not picking up a rescource, because I don't like the idea of it disappearing while I'm away, if my inventory fills up while mining I will get out my flint and steel and incinerate any left on the ground.
I also back up my save files every day in case the computer crashes while saving
Also whenever I post on any forum, if a word looks like it's misspelled I'll google it to make sure (this one isn't in minecraft, but I did it 3 times during this post,)
Don't understand the hate for . If you guys use Smoothstone to patch up alleyways how do you figure out where you've already mined or not? I'm making a Mob Grinder out of Cobble which looks huge and ugly but I just realized it's going to make for a great opportunity for some pixel art (it'll probably end up at least 25x60).
My worst OCD issue at the moment is tending to my farms. I've got more and than I could possibly use but I still insist on going in and harvesting.
My buildings are perfectly symmetrical, even if I didn't intend them to be. And I can't start a new one until I've 100% completed the previous one.. It has to be perfect. And when I put something in the furnace, the number of coal I put in can't be a prime number.

OCD things... Which don't i do?
1) Let's start with slot sickness:
1. Pickaxe (I always need to have a total of 7 stone pickaxes and 2 iron ones with me, no exceptions)
2. Shovels (If i need them - i need to have 3, if i don't - i leave them in one of the thousands of chests everywhere)
3. Axe (-||-)
4. Torches (ALWAYS)
The rest is whatever i need right now, though 5 is usually cobble. Two last ones are always a clock & compass.
2) If i need wood, i chop down the WHOLE tree and punch out all the leaves. I hate it when people leave floating stuff. 3) Every single ore or plant is mine, and i'm taking it with me RIGHT FRIGGIN NOW 4) I avoid entering any caves, because i have to explore it all on the first try 5) Mismatched blocks - Dirt on sand, sand on dirt, stone poking out of a hill - must be replaced. 6) Doors, they need to be closed at all times. 7) Every single building i create is fully equipped with crafting facilities, even if i'm never going to get anywhere near it after buildings. 8) I keep all my resources in one place, even if it means travelling 2 whole ingame days to get there from a mine far away. 9) Full stacks are sacred and not to be touched under any circumstances (Unless i actually need it in mass quantities) 10) I never use gold or diamonds and the only thing i use iron for is the occasional 'lighter' and minetracks. 11) Not even a second of fuel can be wasted when smelting. Even if it means rushing to a beach to get sand just to throw into the furnace. 12) Every wool bearing sheep must be punched and their wool gathered. No exceptions. 13) I don't feel safe if i don't have too much of everything. 14) No mobs are allowed to get inside my fortresses. No exceptions. 15) I never use stair blocks for stairs, half block is the way to go. 16) Artsy beacons everywhere. 17) Every single building needs a name or it doesen't exist 18) Procedures of reed or cacti farming:

2. Get paranoid
3. Whenever it grows a bit, harvest the new block and replant to have moar
4. Repeat until entire horizon is covered with reed/cacti
5. Get angry, harvest the whole farm (leaving 1 reed/cactus behind),

I also always place torches on the right wall regularly when spelunking inward, now that I've learned it's a good habit for navigation-- although I don't know if that counts as OCD. Does it count that, when I find a second cave entrance/exit, I try to adjust nearby torches to lead me towards that entrance when I want to leave (that is, I swap the torches that would lead me there on my right to lead me there on my left, so I can follow them when I'm trying to leave, if that makes sense).
I also try to make a torch path to any cave I find, and mark it with a numbered sign. So the first cave entrance is gotten to via path 1-1, and the second cave's entrance would be 2-1, but if it's the second entrance to the first cave, it'd be 1-2, and so on. To get to portals, the path would be path N1 or H1, depending on what I feel like calling the Nether.

I can't bring myself to throw anything away. We have so many chests scattered throughout the world now its retarded.
I can't bring myself to build the most simple minecart stations now. Everything has to be totally push button, auto reset with in-built minecart dispensing. Its getting waaaay out of control now as I am starting to automate the inventory of the minecart dispensers at the stations to manage undersupply and oversupply. I can't walk past a cave without complete exploration and lighting it up.
When I destroy a mountain, I remove the stone inside first.
I dig up gravel for flint, but keep all the useless gravel in case of a update to use it.
I replace a bunch of gravel and dig it back up, repeat, and get more flint. (Though not much. It takes forever)
I fill in craters from exploded creepers because I think they are ugly.
I like getting a ton of monsters attention and letting the skeletons shoot them all.
Same as above but sometimes they hit a creeper and kills them all.
I like watching monsters fight for me.
I don't use a bow and arrow much, only a sword no matter what the monster.
I hate finding more caves after looking through a large cave system for hours.
-DIRT is think that i allways have (1 stack) i use it to fill areas that going to be replaced, to reach places, block something... and i can't stop filling my stack if it's not full anymore once i get to my chest. REFLEX!
-Tidy mines. I never could make a mess in my mine, there has to be atleast 2x2 widh tunnels with stairs and everything has to be designed even in mines.
-Using stone tools when having about 15 diamonds (not much but well...) and certainly lots of iron (can't even count)
