When I play minecraft on my 360, the mine cart system is messed up. I can't even go a few blocks without falling out of the cart. Just updated it today, too!
Double check to make sure you're not using activator rails. My son did this thinking they were power rails, and they look similar enough that I didn't notice right away. I never use activators myself, so I was pretty confused for a few minutes until I remembered there were rails that did this.
