Is it just me, or is anyone facing the same problem? If you put too many mobs in a certain box, they glitch through the walls (eg cows). Are you all having the same thing, or is it just me?
Entities glitch in through blocks, that is.

Well if you try and stick too many animals in a pen, yes you will find some of them glitch out and when you comeback, your 15 cows are gone with only maybe 2-3 left. It happens also with villagers in Golem farms from time to time.
Try putting water in the corners to force the animals into the center of your pen
Try having fewer animals in each pen and make more pens
This is a well known problem that has been happening in the Xbox 360 Edition as long as I can remember. As stated above, try using flowing water to push the mobs towards the center of your pen or put fewer mobs in a pen. I tend to use relatively small pens with just one breeding pair in each pen.
The glitching through that you see is generally pretty harmless. If left alone (i.e. don't hit them or anything), the mobs that are glitching through will generally "snap" back into the pen after a little bit. Hostile mobs that are glitching through walls and such generally cannot harm you while they are glitching and they too will "snap" back after a little bit of time.
Losses of animals are generally due to the mobs spawning on the wrong side of the fence when the game is reloaded after a save. If you have a lot of mobs in a pen and you happen to save when some are being pushed through or even if the mob is standing on the same block as the fence, the game can then decide to load the mob back in on top of or on the outside of the fence, which can allow it to wander off and/or despawn or suffocate (if, for example, you have a roof on your pens and the animals is spawned on top of the fence). You can also prevent some of it by double fencing - e.g. putting a fence just inside a cobblestone wall or a second, double-high fence, but pushing the mobs to the center of a pen with water seems to work about the best.
