You do sometbing like eat dinner and when you get back you can see that your character is dead. I witnessed this bug a lot. When you pause the game, the entities are still moving and making sound. And time is moving at a normal rate. The bug maybe fixed by the time TU30 comes out into XBOX. Something can still kill you so this bug needs to be fixed.
In a single-player OFFLINE game, pausing the game does stop everything. (I've been paused to type this post... staring at a spider in the middle of an open field for 10 minutes now... and it hasn't moved and I haven't died.) In a splitscreen offline game, one player pausing the game does not stop the "world from turning" (so to speak) because the other players in the world don't want to have their gameplay interrupted if just one of the players wishes to pause. Similarly, in an online enabled game on the Xbox 360 (where there is no physical server being used, but just a host Xbox), the same holds true... others who may be in the world, do not want all their gameplay to stop just because one player in the world wishes to pause for dinner, etc. So, if you are playing alone in your own world, disable the "online game" option and your pauses will stop the game.
If you want to be able to pause your game you need to uncheck the Online box before you load the world. Otherwise, bury yourself in a hole next time you gotta drain it.

I ran a specific test of this as well... my game has paused perfectly and is staying paused even right after doing a manual save (and NOT going in and out of the menu again as you described). The zombie right in front of me that was jumping up and down in the village well when I save is still suspended in mid-air. Could the issue be that you're not re-activating the game properly (i.e. accidentally exiting the pause menu) after your controller shuts down. If your controller has shut down while you are away, use the center X button just long enough to restart the controller without touching any other buttons (the screen will still be dark) then just move your left stick a little bit to brighten the screen. You will see that the game then is STILL paused.
I also have a question, IF you've just done a manual save in a single-player, offline world, would it not be better to just exit the game before leaving for dinner rather than taking the additional time to go in and out of the pause menu again (with no apparent affect anyways). Even better, actually shutting down your console and giving it a rest would be kinder to it (given the Xbox 360's reputation for overheating). Also, if you've just done a manual save and you leave and die, no harm done (as long as you've also turned autosave off)... just exit without saving and reload that manual save.

Any Online or Multiplayer game will not pause.

I've NEVER had this bug. I always play singleplayer cause I don't have anyone else to play with.
