A simple firework is a piece of paper, and a piece of gun powder.
In the crafting table, on the last tab is the ingredients for fireworks
To start:
Choose from 4 items for shape effects. (the 5th choice is nothing)
A fire charge makes a big ball
A gold nugget makes a star
A feather creates a burst
Any mob head is a creeper shape
And no item is a small default ball shape
Adding to this interface, you can a diamond for a trail, or glowstone dust for a twinkling effect.
The last thing you have to add is dye, which you can add a couple colors in for more selection.
You shall now have a firework star.
(Optional) You can now your place custom firework star back the 9 slots. Placing one last dye will create a fade effect. Meaning after your main colors and shape explode, it will fade into any color you choose. You can skin this step, but fade is pretty cool.
You must now place your custom firework star or stars in the 9 slots. The more stars you add, the more craziness and bigger firework you will create!
Lastly, you need 1-3 gunpowder and a piece of paper. Gunpowder is duration, meaning the more gunpowder will make the firework fly higher. When you are finishing placing your stars and gunpowder, place a piece of a paper and safely remove your badass new custom firework.
First you want to open the Firework Crafting Interface. This is a 3x3 grid that allows you drag items into it, similar to crafting on the PC. Only certain items are accepted in this interface; others will be "greyed out."
Now, don't make the mistake of crafting the Rocket first (paper + gunpowder). This makes a firework that simply shoots into the air and vanishes without an explosion. Instead, craft the FW stars. Into the grid place the following:
Gunpowder + dye of your choice
As Gonzo said, you can add additional ingredients into the grid to alter the shape of the explosion.
When you are done, drag it out of the "finish slot" and back into the 3x3 grid. If you want a color fade effect add a different dye color to the grid at this point and remove the new star from the slot.
Now you are ready to craft the actual Rocket.
Place the Star (or Stars if you want multiple shapes contained in a single rocket) into the grid. Add 1 piece of paper and up to 3 gunpowder.
NOTE: When increasing the flight duration, each additional gunpowder must occupy a separate slot in the grid --- "stacked" gunpowder is for crafting multiple rockets at once)
And that's about it.
Just remember: 1 star makes 1 rocket. If you want to make 20 rockets at once, you need 20 stars (all exactly the same), 20 paper, and at least 20 gunpowder.
