Yes, several times and in several different worlds - way back when discs were first introduced. I haven't gone after music discs for a long time, but I can't see why this disc suddenly wouldn't be available now.
It might... IF commands are opened up to the host player at some point in the future... or command blocks get added... but that isn't getting them legitimately... you may as well pull them from the creative menu.
I got the disc legitimately in survival worlds between TU3 and TU7 way back - I stopped playing the game for ages but have started playing again since TU19 and haven't been able to get the disc either - Ive tested this with a mob grinder and for some reason the disc doesnt drop - Ive tried googling but info is sparse - the only clue is I watched a PC Minecraft Lets Play video from 2012 and the uploader mentions that there are 11 discs to collect so perhaps its now been removed from the Xbox version to match the PC - hope this helps but if anyone knows any more Id appreciate it
The music disc is available in the PC version under the title "Wait" rather than "Where are we now.." It was introduced on the Xbox as "Where are we now." but the music is the same. The intrioduction in the PC Edition was in version 1.4. (which is equivalent to the most recent update... so, either the disc got accidentally removed or it has been retitled as "Wait." (which seems more likely)... but I can't confirm it because I'm not updating from TU16 at the current time. Please check to see if you have obtained any discs entitled "Wait." If not, I would suggest you report this on the Official Bugs thread.
I've only gotten approximately 11 discs each of "cat" and "13". I can't seem to find any other disc while finding unreasonable quantities of these two.

Only these two discs are available in chests. To get the others, you have to coerce a skeleton into killing a creeper... then the creeper will drop a disc in addition to the normal gunpowder. You can use different trap arrangements to accomplish this more safely than just out in the open (but it is possible to also do it out in the open my moving about such that you position a creeper between you an a skeleton close enough that he starts shooting at you).
Just dropped it now. It took 38 drops to get all 12 discs

No, it really took 14 months to fix. That was one elusive bug.

No, it really took 14 months to fix. That was one elusive bug.
I can honestly say that I gained maybe a total of five discs in survival during that entire time, so I didn't know or really care. Still.
