I have just started a new survival using Teamwork HD as the seed and it is an ocean biome with a few mushroom island dotted about. This is possibly one of the most enjoyable and challenging survivals I have done. I have accepted that there are some sacrifices to be made here, such as no chickens and a couple of other minor things, but obviously there is also no sugar cane. So, two questions about sugar cane...
Can sugar cane spawn in an area that has already been explored. I think the answer is no, but not sure.
Secondly, has anyone ever found sugar cane in a bonus chest? I managed to get some saplings from the chest and each time it respawned I managed to get some of each variety but as of yet, no sugar cane. I should find some books in a stronghold eventually but until then, no enchantments for me!

You could try getting some zombie villagers and converting them into normal villagers. I'm not sure whether villagers even trade sugar cane, but that is most likely your best bet.
villagers will trade bookcases for emeralds and you could use those directly for your enchantment table or break them to get 3 books from each. I don't believe there is any way for you to get sugar cane. I have never found any in a chest and it will not spawn in explored areas.

True. But unfortunately most librarian trades require paper to unlock the bookcases, and without sugar cane... well.
You need sugar to make a fermented spider eye.....in which to make weakness potions.....in which to cure zombie villagers. Therefore your only bookshelves you can get are from the library in the stronghold. You could wait until the next update. Witches drop sugar i believe. But im not sure if they only spawn in swamps.

To the best of my knowledge, witches only drop sugar, not sugarcane. If none has naturally generated in a world, I know of no way to get it in survival mode.
...and here I thought having almost no trees at all in my default seed was rough... at least I had 3 trees and some sugar cane to get a farm started in my water world... but yeah, no sheep, pigs, or chickens (or ocelots/cats or wolves/dogs) at all. I just have mooshrooms and cows (converted from mooshrooms and since bred) for livestock in my 'Atlantean' underwater village that I'm working on... lost one villager that was converted, finally got another and decided to lock him up until I can get someone for him to breed with, and get the village a bit more defined.
Well...yesterday I found a lone sugar cane at the bottom of the fuh-lippin' ocean growing beside a flooded ravine, so I guess not all hope is lost.
I've seen lone sugar cane growing in caves as well. Not sure, though, if the biome above dictates what sort of plants can spawn in caves and such... but then again, sugar cane can spawn on mushroom islands. Still, best to just count on the books in the stronghold library.
The complete lack of several resources is one reason I like survival island seeds so much... brings the game closer to being about basic survivval.. Long-term survival requires some planning and rationing of scarce resources (like books). I'm currently playing a treeless survival island seed with no bonus chest... relying on whatever mineshafts I find for wood. I know I'll eventually run out completely... but seeing how far I can get before I do is the challenge. It's the only seed I play where I'll use my diamond pickaxe for all mining... since the wooden handle is worth more to me now than the diamonds and if I use iron or stone, I go through too many sticks. The seed also (so far) has no evidence of any sugar cane, no villages, no animals save mooshrooms, and only 3 blades of grass. Finding something unexpectedly after having given up hope of ever obtaining it just seems all the more worthwhile.

With my constantly renewing village, initial trades wanting books and seldomly offering glass/bookshelves do pop up.
