If you have a 1x2 tunnel with a minecart going through it and press left trigger while looking down you glitch through the floor, and on my map you end up falling into lava. Is this a known bug and is there a workaround to it? Will I need to make my tunnels wider? It only happens if you press left trigger to get out, if you mine the cart with right trigger then you are fine.
This almost seems perfectly logical to me... exiting to the side of a mine cart in a tunnel that is only 1 block wide should result in death (I would think it should be by squishing and suffocation though). Maybe report it on the bugs thread.
This is not a bug, but merely a result of exiting the Minecart while in too small of an area. If you would like to not have this happen, you will need to build your minecart tunnels wider.
Making a topic isn't, but there is a thread for it.
