I cannot help you with your main problem. It has never happened to me and I have no idea why it is happening to you.
The bonus chest, however, is an option you must have selected when you created your world. Once a world is created with a bonus chest, the bonus chest will appear each time you start the world. It cannot be turned off after the fact.
If you want a world without a bonus chest, be sure that option is not checked when you first create the world.
Sounds like someone is pushing buttons too quickly. Well that main problem only happens to me on occasion to an extent. It is not launch the first time and will read that message. Of course that happens on occasion with any digital game I play that I have on the hard drive. Usually waiting a moment after pressing ok after that message, and a moment more when loading up the game selection before selecting it to play helps improve my chances to initiate the game.
That really isn't a minecraft glitch, but more a microsoft dashboard glitch that happens from time to time, at least with me. I have seen the game labeled both ways, and will boot up eventually. I have not had that problem persist every time I try though. If you are just pushing buttons quickly to boot up a game; all I can suggest is to slow down a bit and wait for things to load a bit at a time rather than click click click.

Up being an example in that bit being one that has not experienced it. So not all, but seems to affect some in the very least. Whether what I said above is helpful or not, it is how I eventually got in....
