1. Your microphone is always enabled in the game, but if the rest of the players are in a party, you will also have to join the party to talk to them. Disabling your microphone isn't a game setting, either use the mute option on the mic or unplug it from the controller if you want to go silent.
2. On the Xbox, "Hosting a Server" just means that you leave the Xbox that is hosting the world on for extended periods of time. All you have to do is accept friend requests from anyone who wants to join, leave invite only off, and you have an Xbox world that is as open as you can get it.
3. You have to be someone's friend to join their server (or a friend of one of their friends, if the session allows that, e.g. Server Owner Jim is friends with Bob, and Bob is friends with you). They only need to invite you if they have their session set to invite only; otherwise, you will see their game in the lobby (the menu where your save games are).
