If none of your worlds are loading then delete all your skin packs,then you can re-download them.It should work but that is just if all your worlds wont load.You can see if that is the problem by checking your skins in the menu.
Caution: If you delete some of the free ones that were offered only for a limited time, I'm not sure you can necessarily re-download them. For the life of me, I just don't understand this tendency around here to delete the game and it's components every time there is an issue. Deleting your game and components should be one of the last things you try to resolve and issue, not the first.
As Phoenix said, today's issue is due to a problem with Microsoft Live, not Minecraft. You just need to disconnect your Xbox from Live and play offline for the time being until Microsoft fixes the problem.

You can. Everything you have ever downloaded, even if it was taken off the marketplace, can be downloaded again though your Download History. You can find that in your Account Info on the dashboard.

Correct. You can. But I do agree with UpUp_Away that deleting anything should be the last step in troubleshooting, not the first. I feel the same way about clearing the cache, since doing so blows away all the cache-resident game updates (which thankfully MC has gotten away from, going with real files you can see in the MC folder instead). The problem MC is having now at first seemed like corrupt world data, but it is clear now that the problem is a misbehaving Live network and lousy timeout code in MC. Seriously guys, there's no excuse for waiting around longer than 15 seconds or so for a proper response from the network. If it doesn't come in a timely fashion, kick it into offline mode ASAP.
