I dont know if he spawned in the cave but I just saw him floating down a waterfall and it was pretty far down and the waterfall didn't lead to the surface. Sorry I tamed it before taking any photos.
Well I was in a cave one time and I was attacking a zombie when all of a sudden I realized I was attacking a wolf too. I don't think I had ever been in that cave before so I doubt it was one I tamed and accidentally left there. No idea how it got there though.
A couple of weeks ago, I was wandering around underground when I stumbled across my first Stronghold (complete with an intact End Portal). And there was a wolf at the entrance to the Stronghold.

OP - If there Is a waterfall leading from the surface chances are that there Is a Island of any size next to It that spawned dogs.
That Dog then wondered himself into the water probably struggling to swim getting caught into the cave.

hahahaha yesterday i truned around and there was a wold right behind me. they do spawn in caves
